目的探讨碎裂QRS波在心肌梗死临床发病及诊断中的应用效果及价值研究。方法回顾性研究在韶关市第一人民医院经过冠状动脉造影确诊为心肌梗死的100例患者的临床资料,根据患者的心电图QRS波的表现形态分为病理性Q波组、病理性Q波合并碎裂QRS波组、碎裂QRS波组,并根据心肌梗死的不同类型进行统计分析。结果在纳入研究的心肌梗死患者中碎裂QRS波组对发病诊断的敏感性为22.0%,病理性Q波组的敏感性为26.0%,两组比较差异未见统计学意义(P>0.05);碎裂QRS波组的特异性为96.0%,阳性预测值79.0%,高于病理性Q波组,组间比较差异有统计学意义(P<0.05);病理性Q波合并碎裂QRS波组的敏感性为40.0%,高于病理性Q波组和碎裂QRS波组,两组比较差异有统计学意义(P<0.05)。结论破裂性QRS临床上可作为心肌梗死的发病和诊断重要标志物,对临床心肌梗死的发病和诊断具有重要的应用价值。
Objective To investigate the effect and value of fractured QRS wave in the clinical diagnosis and diagnosis of myocardial infarction. Methods The clinical data of 100 patients who were diagnosed as myocardial infarction by coronary angiography in First People’s Hospital of Shaoguan City were retrospectively studied. According to the QRS waveforms of patients, they were divided into pathological Q wave group and pathological Q wave Cracked QRS wave group, fragmented QRS wave group, and according to the different types of myocardial infarction for statistical analysis. Results The sensitivity of the QRS complex was 22.0% and the sensitivity of pathologic Q wave was 26.0% in the patients with MI. There was no significant difference between the two groups (P> 0.05) ; The specificity of fragmented QRS wave group was 96.0%, the positive predictive value was 79.0%, which was higher than that of pathological Q wave group, the difference was statistically significant (P <0.05); pathological Q wave combined with fragmented QRS wave The sensitivity of the group was 40.0%, higher than the pathological Q wave group and fragmented QRS wave group, the difference between the two groups was statistically significant (P <0.05). Conclusion The ruptured QRS can be used as an important marker of myocardial infarction and diagnosis, which has important clinical value in the diagnosis and treatment of clinical myocardial infarction.
