目的 观察VDD起搏器对房室传导阻滞后者的临床运用和工作模式。方法 5例三度房室传导阻滞患者,采用锁骨下静脉穿刺法植入VDD起搏器,并在植入后1周和3月分别做动态心电图(Holter)观察.结果 5例患者均成功植入VDD起搏器,起搏阈值0.6~0.8v,A波感知灵敏度0.3mv.Holter检查显示:VDD起搏器的工作模式大多为VAT,房室同步起搏率达95%以上,偶尔在夜间,P波频率<60bpm时起搏器工作模式自动转换成VVI。结论 VDD起搏器具有VAT和VVI双重功能,对房室传导阻滞患者能增加安全性和改善心脏功能。
Objective To observe the clinical application of VDD pacemaker to atrioventricular block and its working mode. Methods Five patients with third degree atrioventricular block were implanted with VDD pacemaker by subclavian vein puncture. Holter observation was performed at 1 week and 3 months after implantation. Results All 5 patients were successfully implanted with VDD pacemaker. The pacing threshold was 0.6 ~ 0.8v and the sensitivity of A wave was 0.3mv. The Holter test showed that the working mode of the VDD pacemaker was mostly VAT, and the atrial synchronization rate was over 95%. Occasionally, the pacemaker operating mode was automatically converted to VVI at night when the P wave frequency was <60bpm. Conclusion The VDD pacemaker has dual functions of VAT and VVI, which can increase safety and improve cardiac function in patients with atrioventricular block.
