目的观察新诊断2型糖尿病患者在短期胰岛素强化治疗基础上再予中药方剂后续治疗对改善胰岛素抵抗及胰岛β细胞功能恢复的疗效。方法将300例新诊断2型糖尿病患者随机分为观察组和对照组,每组150例。对照组仅给予短期胰岛素强化治疗2周;观察组给予短期胰岛素强化治疗2周基础上后续再予中药方剂治疗3个月。观察并比较两组患者治疗后空腹血糖(FPG)、餐后2 h的血糖(2 hPG)、糖化血红蛋白(HbA_(1c))、血脂、胰岛B细胞分泌功能(HOMA-β)、胰岛素抵抗指数(HOMA-IR)。结果治疗后比较,两组的FPG、2 hPG、HbA_(1c)及HOMA-IR均较本组治疗前有不同程度的降低(均P<0.05),HOMA-β有所上升(均P<0.05);治疗后,观察组的FPG、2 hPG、HbA_(1c)水平的下降程度显著低于对照组(均P<0.05),且观察组HOMA-β升高、HOMAIR下降较对照组更明显(P<0.05)。治疗后,两组患者TC、TG、LDL-C较本组治疗前显著降低,HDL-C较治疗前显著升高(均P<0.05);治疗后,而观察组的TC、TG血脂水平显著的低于对照组(均P<0.05)。结论在短期胰岛素强化治疗基础上再予中药方剂后续治疗,对新诊断2型糖尿病患者胰岛素抵抗改善及胰岛β细胞功能的恢复要比单纯胰岛素短期强化治疗效果好。
Objective To observe the new diagnosis of type 2 diabetes mellitus on the basis of short-term intensive insulin treatment and then follow-up of traditional Chinese medicine prescription to improve insulin resistance and pancreatic β-cell function recovery. Methods 300 newly diagnosed type 2 diabetic patients were randomly divided into observation group and control group, with 150 cases in each group. The control group was given only short-term intensive insulin treatment for 2 weeks. The observation group was treated with short-term intensive insulin treatment for 2 weeks and then treated with traditional Chinese medicine for 3 months. The fasting blood glucose (FPG), postprandial blood glucose (2 hPG), HbA 1c, blood lipid, HOMA-β, insulin resistance index (HOMA-IR). Results After treatment, the levels of FPG, 2 hPG, HbA 1c and HOMA-IR in both groups were significantly lower than those before treatment in both groups (all P <0.05) and HOMA-β ). After treatment, the levels of FPG, 2 hPG and HbA 1c in the observation group were significantly lower than those in the control group (all P <0.05), and the HOMA-β in the observation group was higher than that in the control group P <0.05). After treatment, the levels of TC, TG and LDL-C in both groups were significantly lower than those before treatment, and the levels of HDL-C in the two groups were significantly higher than those before treatment (all P <0.05); after treatment, TC and TG levels in the observation group were significantly Lower than the control group (all P <0.05). Conclusions On the basis of short-term intensive insulin treatment, traditional Chinese medicine prescriptions follow-up treatment is more effective than simple insulin short-term intensive treatment in newly diagnosed type 2 diabetic patients with improved insulin resistance and pancreatic β-cell function.
