目的讨论胃癌的临床诊断及预后判断中血清PGⅠ、PGⅡ的应用价值。方法选取2014年6月—2015年3月间于本院住院治疗的胃癌患者38例为胃癌组,另选同期住院的胃溃疡患者53例(溃疡组),3年前治疗的胃癌术后患者11例(预后组)以及体格健康者63例(对照组),所有参与研究者均进行PGⅠ、PGⅡ、MG-7Ag含量的测定,计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果对照组PGⅠ为(97.63±32.58)ng/ml,溃疡组为(252.32±73.17)ng/ml,两组对比差异有统计学意义(P<0.05);对照组PGⅡ为(37.92±14.12)ng/ml,溃疡组为(32.74±5.92)ng/ml,两组对比差异均无统计学意义(均P>0.05);胃癌组PGⅠ、PGⅡ为(30.97±9.89)、(9.74±6.87)ng/ml,均较对照组低,对比差异均有统计学意义(均P<0.05);预后组为(131.24±49.87)、(24.03±14.32)ng/ml,较胃癌组明显升高,差异均有统计学意义(均P<0.05)。胃癌诊断中,PGⅠ的灵敏度及特异性为78.2%以及93.2%,MG-7Ag为58.3%及82.1%,与MG-7Ag试验相比PGⅠ优势明显。结论在胃癌的临床诊断、疗效观察及预后中,血清胃蛋白酶原水平测定效果均显著于MG-7Ag。
Objective To discuss the clinical value of PGI and PGII in clinical diagnosis and prognosis of gastric cancer. Methods Thirty-eight gastric cancer patients hospitalized in our hospital from June 2014 to March 2015 were selected as gastric cancer group, 53 gastric ulcer patients (ulcer group) hospitalized in the same period, patients with gastric cancer treated three years earlier 11 cases (the prognosis group) and 63 healthy people (control group). All the participants were involved in the determination of PGⅠ, PGⅡ and MG-7Ag. The measurement data were analyzed by t test. The difference was statistically significant. Results The PGI in the control group was (97.63 ± 32.58) ng / ml and in the ulcer group was (252.32 ± 73.17) ng / ml, the difference was statistically significant (P <0.05) / ml and (32.74 ± 5.92) ng / ml in the ulcer group, the difference between the two groups was not statistically significant (all P> 0.05). The expressions of PGⅠ and PGⅡ were (30.97 ± 9.89) and (9.74 ± 6.87) ng / ml, which were lower than those in control group (all P <0.05). The prognosis group was (131.24 ± 49.87) and (24.03 ± 14.32) ng / ml, which was significantly higher than that in gastric cancer group Statistical significance (all P <0.05). The sensitivity and specificity of PGI were 78.2% and 93.2% in gastric cancer diagnosis and 58.3% and 82.1% in MG-7Ag, respectively. Compared with MG-7Ag test, PGI had obvious advantages. Conclusion In the clinical diagnosis, curative effect and prognosis of gastric cancer, the serum pepsinogen level was significantly higher than that of MG-7Ag.
