目的:探讨鼻咽癌放疗后局部复发和纤维变的 CT动态增强特征。材料与方法:放疗后纤维发组(RF组)86例,男54例,女32例。年龄21-62岁。放疗后复发组(RNPC组)52例,男37例,女15例.年龄27岁-68岁。先行鼻咽部平扫,然后选择“兴趣区”行同层动态增强扫描。采用ELSCINT CT-FLASH双螺旋CT扫描机,自动压力注射器注药。分别测量平扫与增强后不同时刻的CT值,进行统计学处理并描绘密度.时间曲线。结果:(1)RF组动态增强曲线呈“缓-升-缓降”型。(2)RNPC组动态增强曲线呈“速升-缓升-缓降”型。(3)注射造影剂后40-300s期间,RF组与RNPC组间各对应时刻点CT值均数均有显著性差异,RNPC组明显高于RF组。结论:RF与RNPC的CT动态增强曲线及病变区CT值的明显差异可作为两者鉴别诊断的新依据。
Objective: To investigate the characteristics of CT dynamic enhancement of local recurrence and fibrosis after radiotherapy for nasopharyngeal carcinoma. Materials and Methods: After radiotherapy, there were 86 cases of RF group (RF group), 54 males and 32 females. Age 21-62 years old. 52 cases of relapse group (RNPC group) after radiotherapy, 37 males and 15 females. Age 27-68 years old. The nasopharynx is scanned first, and then the “interest zone” is selected to perform a dynamic enhancement scan at the same level. Using ELSCINT CT-FLASH double spiral CT scanner, automatic pressure syringe injection. Measure CT values at different times after plain scan and enhancement, and perform statistical analysis and plot density. Time curve. RESULTS: (1) The dynamic enhancement curve of the RF group showed a “slow-rise-decreasing” pattern. (2) The dynamic enhancement curve of the RNPC group showed a “rapid increase-decrease-decrease” type. (3) During the period of 40-300s after injection of contrast medium, there was a significant difference in mean CT values between the RF group and the RNPC group at each time point. The RNPC group was significantly higher than the RF group. Conclusions: The significant difference between the CT dynamic enhancement curves of RF and RNPC and CT values of lesions can be used as a new basis for the differential diagnosis.
