目的探讨经阴道实时三维超声造影(RT 3D-HyCoSy)评价输卵管通畅性的临床应用价值。方法对154例不孕症患者共300条输卵管行RT 3D-HyCoSy检查,观察RT 3D-HyCoSy的显影特征,评估输卵管通畅性,其中79例共152条输卵管与腹腔镜下美兰通液结果进行对照。结果 152条输卵管,RT 3D-HyCoSy诊断输卵管阻塞真阳性32例、假阳性6例、真阴性106例、假阴性8例。诊断敏感性80.0%、特异性94.6%,诊断符合率90.8%。一致性检验Kappa值为0.759(P<0.01),一致程度较好。结论经阴道实时三维子宫输卵管超声造影评价输卵管通畅性安全、简单、可靠,可作为了解不孕症患者输卵管通畅性的主要筛查手段。
Objective To evaluate the clinical value of transvaginal real-time three-dimensional contrast-enhanced ultrasound (RT 3D-HyCoSy) in the evaluation of tubal patency. Methods A total of 300 tubal infertility patients were enrolled in this study. RT-3D-HyCoSy was performed in 154 tubal infertility patients. The development of RT 3D-HyCoSy was observed and tubal patency was assessed. A total of 152 tubal and 79 cases of laparoscopic Control. Results 152 tubal, RT 3D-HyCoSy diagnosis of tubal obstruction in 32 cases, false positive in 6 cases, 106 cases of true negative, false negative in 8 cases. The diagnostic sensitivity was 80.0%, the specificity was 94.6%, and the diagnostic coincidence rate was 90.8%. The consistency test Kappa value of 0.759 (P <0.01), the degree of agreement is better. Conclusion Transvaginal real-time three-dimensional uterine tubal contrast-enhanced ultrasound in the evaluation of tubal patency is safe, simple and reliable, which can be used as a primary screening method to understand tubal patency in patients with infertility.
