目的分析乳腺癌基因(BRCA1)在含基底细胞型乳腺癌组织中的表达,并探讨与其预后因素之间的相关性,了解BRCA1对于患者预后的判断价值。方法回顾性分析142例乳腺癌患者的病历资料,66例确诊的三阴性乳腺癌患者为试验组,其他76例患者为对照组。用免疫组化法检测2组患者肿瘤组织中BRCA1的表达,分析BRCA1在试验组和对照组的表达情况、与基底细胞型乳腺癌临床病理特征之间的关系、与p53和Ki67表达的关系。结果 BRCA1在试验组的阳性表达率为72.70%(48/66),显著高于对照组的55.26%(42/76,P<0.05)。BRCA1与年龄、淋巴结转移情况、组织学分级有关(P<0.05),与民族、肿瘤大小、绝经情况无相关性(P>0.05),其与预后因素p53、Ki67之间存在正相关(P<0.05)。结论 BRCA1是反映肿瘤恶性程度的良好指标,其可作为判断基底细胞型乳腺癌预后的指标,为临床个体化治疗提供依据及新的治疗方向。
OBJECTIVE: To analyze the expression of breast cancer gene (BRCA1) in basal cell-bearing breast cancer and explore the correlation with BRCA1 prognosis. Methods A retrospective analysis of 142 cases of breast cancer patient records, 66 cases of triple negative breast cancer patients as the experimental group, the other 76 cases of the control group. The expression of BRCA1 in the tumor tissue of the two groups was detected by immunohistochemistry. The relationship between the expression of BRCA1 in the experimental group and the control group, the clinicopathological features of the basal cell carcinoma and the expression of p53 and Ki67 were analyzed. Results The positive expression rate of BRCA1 in the experimental group was 72.70% (48/66), which was significantly higher than that in the control group (55.26%, 42/76, P <0.05). There was no significant correlation between BRCA1 and age, lymph node metastasis and histological grade (P <0.05), but no correlation with ethnicity, tumor size and menopause (P> 0.05). There was a positive correlation between BRCA1 and prognosis factors p53 and Ki67 (P < 0.05). Conclusion BRCA1 is a good indicator to reflect the malignant degree of the tumor, which can be used as an index to judge the prognosis of BCC. It provides a basis for clinical individualized treatment and a new treatment direction.
