目的 :探讨胃肠道间质瘤 (GISTs)的螺旋CT表现以及螺旋CT对胃肠道间质瘤的诊断价值。方法 :搜集经手术病理证实的胃肠道间质瘤 3 5例 ,均行螺旋CT平扫及双期增强扫描。结果 :3 5例中 ,食管间质瘤 1例 ,胃间质瘤 2 4例 ,肠道间质瘤 10例。GISTs的主要螺旋CT表现为 :胃肠道软组织肿块 ,向腔内、外或腔内外突出 ,肿块内密度均匀或呈混杂密度影。增强后 ,肿块强化较明显或呈中心坏死、囊变 ,周边强化。 3 5例GISTs中 ,良性 7例 ,肿块直径多小于 5mm ,边界清楚 ,多均匀强化 ;恶性 2 8例 ,肿块直径大于 5mm ,边界多不清楚 ,17例肿块内有坏死 ,6例出现转移灶。结论 :螺旋CT增强扫描显著提高了GISTs的检出率和诊断准确性 ,弥补了常规胃肠道造影和内窥镜检查的不足 ,对GISTs的定位诊断和鉴别诊断有重要价值
Objective: To investigate the spiral CT findings of gastrointestinal stromal tumors (GISTs) and the diagnostic value of spiral CT in gastrointestinal stromal tumors. Methods: Thirty five cases of gastrointestinal stromal tumors confirmed by surgery and pathology were collected. All patients underwent spiral CT scan and double-phase enhanced scan. Results: In 35 cases, 1 case of esophageal stromal tumor, 24 cases of gastric stromal tumor and 10 cases of intestinal stromal tumor. GISTs showed the main spiral CT: gastrointestinal soft tissue mass, protruding to the inside, outside or inside and outside the cavity, the mass density is uniform or mixed density shadow. Enhanced, the more obvious tumor mass enhancement or central necrosis, cystic degeneration, peripheral enhancement. In 35 cases of GISTs, benign in 7 cases, the diameter of the tumor was less than 5mm, the boundary was clear and more uniform enhancement; malignancy in 28 cases, the tumor diameter was greater than 5mm, the border was unclear, 17 cases had necrosis and 6 cases had metastases . Conclusion: Spiral CT enhanced scan significantly improves the detection rate and diagnostic accuracy of GISTs, which can make up for the shortcomings of conventional gastrointestinal angiography and endoscopy, and is of great value in the diagnosis and differential diagnosis of GISTs
