目的观察星状神经阻滞对偏头痛的治疗效果及阻滞后30 min阻滞侧脑血流的变化。方法随机选择偏头痛患者40例,经头颅CT扫描排除颅内器质性病变。作单侧星状神经阻滞,采用气管旁接近法,阻滞成功与否,以出现Hornev’s征为标志。在阻滞前与阻滞完成后30 min,用经颅多普勒诊断仪测定阻滞侧脑血流变化,经统计学处理t检验。结果阻滞侧脑血流变化明显,与阻滞前比较血流增加,P<0.01。结论星状神经阻滞后可明显增加脑血流量,改善脑血管痉挛。阻断偏头痛发作的恶性循环,治疗效果肯定且安全,可在临床使用。
Objective To observe the therapeutic effect of stellate ganglion on migraine and the changes of the blocked side cerebral blood flow 30 minutes after block. Methods Forty patients with migraine were randomly selected and skull CT scans were performed to exclude intracranial organic diseases. For unilateral stellate block, the use of tracheal approach, the success or failure of block, in order to appear Hornev’s sign as a sign. In the block before and after the completion of the block 30 min, with transcranial Doppler diagnostic apparatus to determine the changes of cerebral blood flow, the statistical analysis of t test. Results Obstructive lateral cerebral blood flow changes significantly, compared with pre-block blood flow increased, P <0.01. Conclusion Stellate blockade can significantly increase cerebral blood flow and improve cerebral vasospasm. Block the vicious cycle of migraine attacks, the treatment effect of certain and safe, can be used clinically.
