目的观察奈达铂(Nedaplatin)联合氟尿嘧啶(5-Fluorouracil,5-Fu)、四氢叶酸钙(Leu-covorin,CF)组成的方案(观察组),与顺铂(Cisplatin,DDP)联合氟尿嘧啶、四氢叶酸钙组成的方案(对照组)治疗晚期食管癌的近期疗效及毒副反应。方法对68例经病理或细胞学证实的晚期食管癌患者给予联合化疗,观察组36例,对照组32例,两组病例具有可比性。奈达铂100mg/m2,静脉滴注,第1天;氟尿嘧啶375mg/m2,静脉滴注,第1~5天;四氢叶酸钙200mg/m2,静脉滴注,第1~5天;顺铂25mg/m2,静脉滴注,第1~3天。21d为1周期,每例患者治疗3周期。结果观察组总有效率55.6%,对照组总有效率46.9%,差异无显著性(P>0.05);对照组胃肠道毒性和肾毒性均显著高于观察组(P<0.05)。结论顺铂联合方案与奈达铂联合方案治疗晚期食管癌近期疗效相似,但顺铂联合方案胃肠道毒性及肾毒性均显著高于奈达铂联合方案。
Objective To observe the effects of Nedaplatin combined with 5-Fluorouracil (5-Fu) and Leu-covorin (CF) in the observation group and Cisplatin combined with fluorouracil, Calcium leucovorin (control group) for the treatment of advanced esophageal cancer in the short term efficacy and side effects. Methods Sixty-eight patients with advanced esophageal cancer confirmed by pathology or cytology were given chemotherapy combined with 36 cases in observation group and 32 cases in control group. The two groups were comparable. Nedaplatin 100mg / m2, intravenous infusion, the first day; fluorouracil 375mg / m2, intravenous drip, the first 5 days; leucovorin 200mg / m2, intravenous drip, 1st to 5th days; 25mg / m2, intravenous infusion, 1 to 3 days. 21d for a cycle, each patient treatment for 3 cycles. Results The total effective rate was 55.6% in the observation group and 46.9% in the control group, with no significant difference (P> 0.05). The gastrointestinal toxicity and nephrotoxicity in the control group were significantly higher than those in the observation group (P <0.05). Conclusion The cisplatin combination regimen and nedaplatin combination regimen have similar efficacy in the treatment of advanced esophageal cancer. However, the cisplatin combination regimen has significantly higher gastrointestinal toxicity and nephrotoxicity than the nedaplatin combination regimen.
