目的:探讨联合抗心磷脂抗体(ACA)和子宫动脉、胎儿脐动脉搏动指数(PI)、阻力指数(RI),脐动脉S/D变化在预测重度子痫前期新生儿低体重和新生儿窒息等不良结局方面的临床意义。方法:检测2012年1月至2012年6月间住院剖宫产分娩的115例晚期妊娠重度子痫前期孕妇血清ACA,应用彩色多普勒超声检测孕妇子宫动脉和胎儿脐动脉血流的PI、RI和S/D值,与新生儿体重和脐血PH值进行比较分析。结果:重度子痫前期孕妇中ACA阳性组与ACA阴性组比较,子宫动脉和胎儿脐动脉血流参数均显著升高;ACA阳性重度子痫前期孕妇新生儿体重和脐血PH值均显著低于ACA阴性孕妇;ACA阳性重度子痫前期孕妇胎儿脐动脉S/D与新生儿体重及脐血PH值均呈负相关(P<0.05)。结论:联合ACA和彩色多普勒超声预测重度子痫前期新生儿预后具有重要临床意义。
OBJECTIVE: To investigate the relationship between ACA and uterine arteries, fetal umbilical artery pulsatility index (PI), resistance index (RI) and umbilical artery S / D changes in the prediction of low birth weight and neonatal asphyxia in severe preeclampsia And other adverse clinical outcomes. Methods: Serum ACA levels in 115 pregnant women with severe preeclampsia who were hospitalized for cesarean delivery between January 2012 and June 2012 were detected. Results: The blood flow parameters of uterine artery and fetal umbilical artery were significantly increased in ACA-positive pregnant women with severe preeclampsia compared with those in ACA-negative pregnant women. The weight of neonates and PH in cord blood of pregnant women with severe preeclampsia were significantly lower than those of ACA- ACA negative pregnant women; ACA-positive pregnant women with severe preeclampsia fetal umbilical artery S / D and neonatal body weight and cord blood PH value was negatively correlated (P <0.05). Conclusion: The combination of ACA and color Doppler ultrasound in predicting the prognosis of severe preeclampsia has important clinical significance.
