辨证分型联合西药治疗小儿喘息性疾病随机平行对照研究

[目的]观察辨证分型联合西药治疗小儿喘息性疾病疗效。[方法]使用随机平行对照方法,将102例住院及门诊患者按抛硬币法简单随机分为两组。对照组5l例抗炎抗病毒、止咳化痰平喘,吸氧、镇静等支持治疗,0.5%沙丁胺醇吸入液0.5mL+0.025%异丙托溴胺吸入液+普米克令舒,氧气驱动雾化,氧流量6L·min~(-1),吸入5~8min/次,2次/d。治疗组51例辨证分型,邪犯肺卫:风热闭肺(生石膏~(先煎)24g,连翘、金银花各10g,射干、丹参、僵蚕各6g,杏仁、桃仁、地龙各5g,麻黄3g,生甘草2g);风寒闭肺(陈皮、苏子、法半夏、杏仁各8g,地龙、甘草、葶苈子、桔梗各6g,麻黄炙5g,细辛1g);痰浊内蕴:寒痰-活阳活血化瘀汤(茯苓、法半夏、丹参、附片、杏仁、桃仁各6g,地龙、桂枝、僵蚕、五味子各5g,干姜、细辛、生麻黄、甘草各3g),热痰-清热化痰汤(生石膏15g,川贝母、瓜萎仁各10g,杏仁、前胡、黄芩、苏子各6g,莱菔子5g);肾虚失纳(紫石英、五味子、丹参、淫羊藿、杏仁、巴戟天、桃仁各6g,地龙、僵蚕各5g,红花3g,麻黄~炙、生麻黄各2g;病情危重可先予生晒参、附片各6g,蛤蚧1对,浓煎鼻饲频服,病情渐稳后予上方);均1剂/d,水煎200mL,早晚口服,100mL/次;西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、症状消失时间、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效46例,有效5例,无效0例,总有效率100.00%。对照组显效30例,有效17例,无效4例,总有效率92.16%。治疗组疗效优于对照组(P<0.05)。症状消失时间治疗组改善优于对照组(P<0.01)。[结论]辨证分型联合西药治疗小儿喘息性疾病疗效满意,无严重不良反应,值得推广。 [Objective] To observe the curative effect of syndrome differentiation and western medicine on asthmatic children. Continuous treatment 7d for a course of treatment. Observation of clinical symptoms, symptoms disappear time, adverse reactions. Treatment of a course of treatment to determine the efficacy. [Results] In the treatment group, 46 cases were markedly effective, 5 cases effective, 0 ineffective, and the total effective rate was 100.00%. The control group, markedly effective in 30 cases, effective in 17 cases, 4 cases, the total effective rate was 92.16%. The treatment group was better than the control group (P <0.05). Symptoms disappeared in the treatment group improved better than the control group (P <0.01). [Conclusion] The syndrome differentiation and western medicine treatment of children with wheezing disorders have satisfactory curative effect and no serious adverse reactions, which deserves promotion.
