目的研究比较Er,Cr:YSGG激光与传统牙钻均分别预备老年及青年离体磨牙Ⅴ类洞对复合树脂充填微渗漏的影响。方法于2012年5月~9月,在中国医科大学口腔医院,将10颗老年及10颗青年因牙周病或阻生拔出的无龋坏、无充填物、无隐裂的磨牙,按预备方式不同,平均分为四组:第一组,老年高速;第二组,老年激光;第三组,青年高速;第四组青年激光,每组5颗牙。经200次冷热循环染色后,将上述所有样本置于0.5%碱性品红溶液中浸泡24小时后,沿牙体长轴通过充填体正中纵行剖开,在体视显微镜下根据染料渗入等级评分(0~3)及扫描电镜方法观察充填体微渗漏情况。结果牙合壁和龈壁处,染料渗入结果显示,老年组和青年组染料渗入评分差异有统计学意义(P<0.05),老年组大于青年组;激光组和牙钻组差异无统计学意义(P>0.05)。充填体边缘微缝隙宽度的结果显示,4组充填体边缘微缝隙宽度差异存在统计学意义(P<0.05),其中老年牙钻组的边缘微裂隙最大,青年激光组的边缘微裂隙最小。结论在自酸蚀粘结剂的应用条件下,Er,Cr:YSGG激光预备处理Ⅴ类洞不能显著减少光固化复合树脂边缘微渗漏的发生;老年磨牙组与青年磨牙组相比较,无论是Er,Cr:YSGG激光还是牙钻预备光固化复合树脂充填后,微渗漏评分及边缘微缝隙均大于青年磨牙组。
Objective To compare the effects of Er, Cr: YSGG laser and traditional dental drill on the microleakage of composite resin filled Ⅴ-type holes in aged and young patients respectively. Methods From May to September 2012, at the Stomatological Hospital of China Medical University, 10 aged and 10 young people who had no caries, no filling and no cracked molars due to periodontal disease or impaction were treated with Different preparation methods, the average is divided into four groups: the first group, elderly high-speed; the second group, the elderly laser; the third group, young high-speed; fourth group of young laser, each group of five teeth. After 200 cycles of hot and cold cycles, all the above samples were soaked in 0.5% alkali fuchsine solution for 24 hours, then cut along the longitudinal axis of the tooth body through the middle longitudinal longitudinal section of the filling body, and were observed under a stereomicroscope for dye penetration Grading (0 ~ 3) and scanning electron microscopy method to observe the microleakage of the filling body. Results At the wall of the occlusal wall and the gingival wall, the results of dye infiltration showed that there was a significant difference in dye infiltration score between the aged group and the youth group (P <0.05), the aged group was larger than the youth group, and the difference between the laser group and the dental drill group was not statistically significant (P> 0.05). The results showed that there were significant differences in the widths of the micro-gap between the four groups (P <0.05), among which, the micro-cracks in the edge group of the old teeth group were the largest and the micro-cracks in the edge group of the young laser group were the smallest. Conclusions In the application of self-etching adhesives, Er-Cr: YSGG laser pre-treatment of Ⅴ-type cavities can not significantly reduce the occurrence of microleakage at the edge of the light-cured composite resin. Compared with the young molars group, After the Er, Cr: YSGG laser or dental laser pre-prepared light-cured composite resin filled, the microleakage score and the edge micro-cracks were larger than the young molar group.
