目的分析核磁共振成像(MRI)检查对于妊娠期胎盘植入(PIA)的诊断价值以及对妊娠期胎盘植入与妊娠结局的相关性。方法选取87例疑似妊娠期胎盘植入患者作为观察组,另选同期52例正常健康孕妇作为对照组。观察组孕妇于产前行MRI检查,以分娩病理结果为金标准,评估MRI诊断妊娠期胎盘植入的价值;比较两组孕妇的妊娠结局,并分析胎盘植入与妊娠结局的相关性。结果 MRI诊断的敏感度为91.23%,特异度为83.33%,诊断符合率为88.51%,与金标准具有一致性,差异具有统计学意义(Kappa=0.746,P<0.05)。对照组剖宫产率为15.38%,不良妊娠结局发生率为28.85%;观察组剖宫产率为98.85%,不良妊娠结局发生率为49.43%;观察组剖宫产率、不良妊娠结局发生率均高于对照组,差异具有统计学意义(P<0.05)。胎盘植入与剖宫产发生呈正相关(r=0.863,P<0.05),与不良妊娠结局呈正相关(r=0.363,P<0.05)。结论产前MRI诊断妊娠期胎盘植入具有较高的诊断准确性,可为临床诊断提供数据性指导,本病与不良妊娠结局具有相关性。
Objective To analyze the diagnostic value of magnetic resonance imaging (MRI) in the diagnosis of placenta accreta (PIA) in pregnancy and its relationship with the outcome of pregnancy and placenta accreta in pregnancy. Methods Eighty-seven patients with suspected placenta accreta in pregnancy were selected as the observation group. Another 52 healthy pregnant women were selected as the control group during the same period. The observation group of pregnant women in prenatal line MRI examination to the delivery of pathological results as the gold standard to assess the value of MRI diagnosis of placenta accreta in pregnancy; pregnancy outcomes were compared between the two groups, and analysis of placenta accreta and pregnancy outcome. Results The diagnostic sensitivity of MRI was 91.23%, the specificity was 83.33%, the diagnostic coincidence rate was 88.51%, which was consistent with the gold standard. The difference was statistically significant (Kappa = 0.746, P <0.05). The rate of cesarean section in the control group was 15.38%, the incidence of adverse pregnancy outcomes was 28.85%; the rate of cesarean section in the observation group was 98.85% and the incidence of adverse pregnancy outcomes was 49.43%; The incidence of cesarean section and adverse pregnancy outcome in the observation group Both higher than the control group, the difference was statistically significant (P <0.05). There was a positive correlation between placenta accreta and cesarean section (r = 0.863, P <0.05), which was positively correlated with adverse pregnancy outcomes (r = 0.363, P <0.05). Conclusion Prenatal MRI diagnosis of placenta accreta in pregnancy has high diagnostic accuracy, which can provide data for clinical diagnosis guidance, the disease is associated with adverse pregnancy outcomes.
