目的:对妊娠糖尿病患者脂联素水平及其与胰岛素抵抗现象之间的相关性进行分析。方法:选择2013年4月至2016年4月本院收治的获得明确诊断的妊娠糖尿病患者76例作为观察组,另取同期正常妊娠孕妇76例作为妊娠对照组;选择同期健康未孕妇女76例作为健康对照组,对3组对象的脂联素、炎性因子等进行检测,并分析其与胰岛素抵抗的关系。结果:观察组患者脂联素水平(7.8±5.2)mg/L,C反应蛋白水平(11.1±4.2)mg/L,胰岛素抵抗指数(2.9±0.5);妊娠对照组孕妇脂联素水平(14.5±3.2)mg/L,C反应蛋白水平(6.3±2.1)mg/L,胰岛素抵抗指数(1.9±0.5);健康对照组妇女脂联素水平(18.7±4.9)mg/L,C反应蛋白水平(3.3±1.1)mg/L,胰岛素抵抗指数(1.2±0.6),3组受试者的脂联素、C反应蛋白以及胰岛素抵抗指数等相关指标进行比较,组间比较,差异具有统计学意义(P<0.05);妊娠阶段的妇女脂联素的实际水平与炎性因子、胰岛素抵抗指数之间也存在明显的负相关性(P<0.05)。结论:脂联素、炎性因子、胰岛素抵抗之间存在密切的相关性,在今后的临床诊断和治疗工作中,应对其给予足够的重视。
Objective: To analyze the correlation between adiponectin level and insulin resistance in gestational diabetes mellitus. Methods: From April 2013 to April 2016, 76 cases of gestational diabetes mellitus with definite diagnosis in our hospital were selected as the observation group. Another 76 pregnant women of the same period as the pregnancy control group were selected as the control group. 76 healthy pregnant women As a healthy control group, three groups of subjects were tested for adiponectin, inflammatory factors and the like, and their relationship with insulin resistance was analyzed. Results: The levels of adiponectin (7.8 ± 5.2) mg / L, C reactive protein level (11.1 ± 4.2) mg / L and insulin resistance index (2.9 ± 0.5) ± 3.2) mg / L, the level of C-reactive protein (6.3 ± 2.1) mg / L and insulin resistance index (1.9 ± 0.5). The level of adiponectin in healthy controls was (18.7 ± 4.9) (3.3 ± 1.1) mg / L, insulin resistance index (1.2 ± 0.6), adiponectin, C-reactive protein and insulin resistance index of the three groups were compared, the difference was statistically significant (P <0.05). There was also a significant negative correlation between the actual levels of adiponectin and the inflammatory factors and insulin resistance index in pregnant women (P <0.05). Conclusion: There is a close correlation between adiponectin, inflammatory factors and insulin resistance. In the future clinical diagnosis and treatment, it should be given enough attention.
