Stroke is the third leading cause of death in adults worldwide.Worldwide, about 5.6 million people died from stroke in 2005, according to the data from WHO.Cerebral ischemia is caused by a reduction of the cerebral blood flow leading to a cascade of pathological mechanisms such as loss of ionic homeostasis, energy failure,excessive release of excitatory amino acids, increased oxidative stress, apoptosis and finally irreversible tissue damage.Neuroprotective agents acting on individual neurotoxic mechanisms have been tested with promising success in animal models, but all of them have failed to benefit patients in clinical trials, for less considering the integriy of brain function.The National Institutes of Neurological Disorders and Stroke convened the Stroke Program Review Group to review the basic and clinical research on stroke in the past 20 years, advise on directions for basic and clinical stroke research for the next decade, and emphasized the relevance of dynamic interactions between endothelial cells, vascular smooth muscle, astroglia and microglia, neurons and associated tissue matrix proteins and its importance to disease pathophysiology.According to this, since 2006, we proposed a series of specific targets for brain injury repairment and treatment for ischemia stroke, and investigated the neuroprotective effects of MNG on ischemic stroke, which also further support our hypothesis.The research of MNG has been supported by 10 national and municipal science foundations and 11 postgraduates have participated in.The data presently indicated that MNG could increase the expression of neurotrophic factors, promote neurogenesis and angiogenesis detected by double immunofluorescence, leading to the increase of the number of neurons and vessels, could also inhibit inflammatory factors and regulate cerebrovascular permeability for BBB repairment,finally improve neurological function.Associated mechenisms may be involved such as Wnt/β-catenin pathway.This work showed the neuroprotective effects of MNG against the ischemia-induced injury on neural and vascular composition.It will be useful towards the development of novel therapeutic strategies for the treatment of ischemic stroke.
