【目的】探讨短棒状杆菌抑制剂胸腔内注射对晚期肺癌胸腔积液患者的疗效及对外周血γ‐干扰素(IFN‐γ)、白细胞介素‐4(IL‐4)、T淋巴细胞亚群的影响。【方法】选择2012年1月至2013年12月于本院接受治疗的晚期肺癌合并恶性胸腔积液患者84例为研究对象,随机均分为观察组和对照组各42例,均行中心静脉导管持续引流,同时前者行短棒状杆菌制剂胸腔内注射;后者行顺铂胸腔内注射。酶联免疫吸附试验(ELISA )检测患者外周血 IFN‐γ及 IL‐4水平,免疫组化法检测外周血 T 淋巴细胞亚群CD4+、CD8+水平,并比较两组疗效。【结果】观察组完全有效(C R )16例,占38.1%、总有效率90.5%(38/42),显著性高于对照组总有效率45.2%(19/42),其差异有统计学意义( P <0.05);两组患者治疗后IFN‐γ、CD4+及CD4+/CD8+水平均较治疗前显著性上升,IL‐4及CD8+水平均较治疗前显著性下降( P <0.05),且观察组治疗后数据与对照组相比,差异亦有统计学意义( P <0.05)。【结论】短棒状杆菌制剂胸腔内注射有助于提升晚期肺癌合并胸腔积液患者疗效,并纠正免疫系统紊乱,值得推广。“,”Objective]To explore the efficacies of intrathoracic injection of short corynebacterium parvum in‐hibitors for immune functions of advanced lung cancer patients with pleural effusion with and influence of peripher‐al bloodγ‐interferon (IFN‐γ) ,interleukin‐4 (IL‐4) and .[Methods]A total of 84 patients of advanced lung cancer with malignant pleural effusion at our hospital between January 2012 and December 2012 were randomly divided in‐to observation and control groups ( n=42 each) .All underwent continuous central venous catheter drainage .The observation group received an intrathoracic injection of short corynebacterium preparation while the control group had an intrathoracic injection of cisplatin .Enzyme‐linked immunosorbent assay (ELISA) was employed to detect the peripheral blood levels ofγ‐interferon (IFN‐γ) and interleukin‐4 (IL‐4) .And immunohistochemistry was used for detecting the peripheral blood levels of CD4+ T and CD8+ T .The efficacies were compared statistically .[Re‐sults]Among them ,16 cases (38 .1% ) of observation group achieved complete remission (CR) with an overall ef‐fective rate of 90 .5% (38/42) .And it was significantly higher than that of control group [11 .9% ,45 .2% (19/42)] .And the differences were statistically significant ( P<0 .05) .As compared with pre‐therapy ,the post‐ther‐apy levels of IFN‐γ ,CD4+ and CD4+ /CD8+ significantly increased while those of IL‐4 and CD8+ significantly de‐creased ( P<0 .05) .And as compared with control group ,the post‐therapy differences were also statistically sig‐nificant ( P<0 .05) .[Conclusion]For advanced lung cancer patients ,an intrathoracic injection of short corynebac‐terium preparation may boost efficacies and correct immune system disorders .It is worth wider popularization .
