1 病例摘要患者女性,75岁.因反复胸闷、心悸、头痛20年,烦躁不安半天,于1990年1月2日入院.有高血压史20年,曾不规则服降压药片.近10年来心悸加剧,心电图示房颤,持续存在.入院前24天起胸闷心悸加剧,尿量减少伴下肢浮肿,口服10%氯化钾液每日30ml,双氢克尿噻每日12.5mg.体检:T37.5℃,P16,BP10.6/6.6kPa.瞳孔无殊.口唇轻度紫绀.两肺底细湿罗音,心左界略大,心率16,心律略不齐,心尖区Ⅱ/6
1 case summary Female patients, age 75. Due to repeated chest tightness, palpitations, headache for 20 years, restlessness for half a day, was admitted to hospital on January 2, 1990. Hypertensive history of 20 years, had irregular service antihypertensive tablets. Heart palpitations, ECG atrial fibrillation, persistence. 24 days before admission increased chest tightness palpitations exacerbation, decreased urine output with lower extremity edema, oral 30% potassium chloride daily 30ml, hydrochlorothiazide 12.5mg daily. Physical examination: T37.5 ℃, P16, BP10.6 / 6.6kPa. Pupil without special lips mild cyanosis. Two lungs at the end of fine wet rales, slightly left heart heart rate, heart rate 16, slightly irregular heartbeat, apex Ⅱ / 6
