目的:探讨Survivin蛋白在喉鳞状细胞癌(LSCC)组织中的表达及其相关性。方法:采用免疫组织化学SP法检测64例LSCC、28例正常喉黏膜及20例喉良性病变组织中的Survivin、Bcl2蛋白的表达,结合临床病理特征进行统计分析。结果:64例LSCC组织中,Survivin蛋白表达阳性43例;28例正常喉黏膜和20例喉良性病变中分别表达1例和2例。Survivin蛋白在声门上型LSCC中表达32例,声门型中表达11例;中低分化的标本中表达34例,高分化的标本中表达9例;在临床分期的Ⅲ、Ⅳ期标本中表达33例,在Ⅰ、Ⅱ期标本中表达10例。以上各项指标之间差异具有统计学意义(均P<0.05)。在有淋巴结转移的标本中表达30例,无淋巴结转移的标本中表达13例,两者之间差异具有统计学意义(P<0.01)。在LSCC组织中Survivin与Bcl2蛋白表达有明显相关性(P<0.05)。结论:Survivin过度表达提示LSCC临床分期晚、分化程度低、易发生淋巴结转移,对于LSCC的早期诊断和预防具有重要参考意义。Bcl2与Survivin的表达可能在LSCC的发病机制中起协同作用。
Objective: To investigate the expression of Survivin protein in laryngeal squamous cell carcinoma (LSCC) and its correlation. Methods: The expressions of Survivin and Bcl2 protein in 64 cases of LSCC, 28 cases of normal laryngeal mucosa and 20 cases of laryngeal benign lesions were detected by immunohistochemical SP method. Statistical analysis was made based on clinicopathological features. RESULTS: Among 64 LSCC tissues, 43 were positive for Survivin protein expression, and 1 and 2 were found in 28 normal laryngeal mucosa and 20 laryngeal benign lesions respectively. Survivin protein was expressed in supraglottic LSCC in 32 cases and in glottis in 11 cases, in poorly differentiated samples in 34 cases and in well differentiated samples in 9 cases. Survivin protein was found in stage Ⅲ and Ⅳ of clinical stage 33 cases were expressed, and 10 cases were expressed in stage Ⅰ and Ⅱ. The differences between the above indicators were statistically significant (P <0.05). Thirty cases were expressed in the specimens with lymph node metastasis and 13 cases were not detected in the cases with lymph node metastasis, the difference was statistically significant (P <0.01). There was a significant correlation between Survivin and Bcl2 protein expression in LSCC (P <0.05). Conclusion: Overexpression of Survivin suggests that the clinical stage of LSCC is late, the degree of differentiation is low and lymph node metastasis is prone to occur, which has important reference value for the early diagnosis and prevention of LSCC. The expression of Bcl2 and Survivin may play a synergistic role in the pathogenesis of LSCC.
