目的:探讨细胞核形态学参数在腮腺多形性腺瘤恶性增殖发生、发展中的生物学意义及临床指导意义。方法:采用流式细胞术对腮腺多形性腺瘤进行肿瘤细胞群体的增殖周期动力学检测,利用计算机图象处理系统进行肿瘤细胞核形态计量,根据肿瘤细胞核的形态测量参数和导出参数等相关数据,进行统计学分析处理。结果:从良性多形性腺瘤到多形性腺瘤增生活跃型、恶性多形性腺瘤,随肿瘤细胞增生活跃程度增加,SPF及PI指数逐渐增高,组间差异有统计学意义,P=0.00;肿瘤细胞核面积、周长、最长径和最短径均为依次递增,肿瘤细胞核形状因子、不规则指数均依次增大。多形性腺瘤增生活跃型与恶性多形性腺瘤在周长、最长径、最短径、形状因子和不规则指数方面相比差异无统计学意义,P>0.05。结论:肿瘤细胞核形态学测量参数在判断肿瘤恶性增殖方面有重要意义,对于早期确定有高恶变可能的多形性腺瘤病变有重要的参考价值。
Objective: To investigate the biological significance and clinical significance of nuclear morphological parameters in the occurrence and development of malignant proliferation of parotid pleomorphic adenoma. Methods: The proliferative cycle kinetics of tumor cell population of parotid pleomorphic adenoma was detected by flow cytometry. The nuclear morphology of tumor cells was measured by computer image processing system. According to the morphometric parameters and derived parameters of tumor nuclei, Statistical analysis and processing. Results: From benign pleomorphic adenoma to pleomorphic adenoma proliferating active type and malignant pleomorphic adenoma, the SPF and PI index gradually increased with the increase of tumor cell hyperplasia, the difference was statistically significant (P = 0.00); The nucleus area, perimeter, longest diameter and shortest diameter of tumor cells all increased in sequence, and the shape factors of tumor nuclei and irregularity index increased in sequence. Polymorphic adenoma hyperplasia active and malignant pleomorphic adenoma in the circumference, the longest diameter, the shortest diameter, shape factor and irregular index compared to no significant difference, P> 0.05. Conclusion: The measurement parameters of tumor cell nuclear morphology are of great significance in judging the malignant proliferation of tumor. It is an important reference value for the early determination of pleomorphic adenoma with high malignant potential.
