目的探讨老年糖尿病患者餐后三酰甘油(TG)升高对血管内皮功能的影响。方法回顾性分析2013年1月至2014年1月大连市友谊医院老年病科收治的老年糖尿病患者资料,其中50例为观察组,检测其餐前及餐后2、4、6h的TG水平,并采用超声检测空腹颈动脉内-中膜厚度(IMT)及脂肪餐前后血管内皮依赖性舒张功能(FMD),同时选取非糖尿病老年患者50例作为对照组。结果观察组患者餐后各时点TG水平明显比餐前增高,差异均有统计学意义(均P<0.05);观察组各时点TG水平均明显高于对照组,差异均有统计学意义(均P<0.05);对照组餐后2 h、4 h的TG水平高于餐前,差异有统计学意义(P<0.05),但餐后6 h恢复到正常水平。观察组空腹时颈动脉IMT明显高于对照组,差异有统计学意义(P<0.05);观察组FMD明显低于对照组,差异有统计学意义(P<0.05);观察组与对照组餐后4 h时颈动脉IMT比较,差异无统计学意义(P>0.05);FMD较空腹时均明显降低,差异有统计学意义(P<0.05)。结论老年糖尿病患者餐后TG升高会对血管内皮功能产生影响,可以促使糖尿病合并心血管疾病的发生。
Objective To investigate the effect of postprandial triglyceride (TG) on vascular endothelial function in elderly diabetic patients. Methods The data of elderly diabetic patients admitted to the Department of Geriatrics, Dalian Friendship Hospital from January 2013 to January 2014 were retrospectively analyzed. Among them, 50 cases were in the observation group, and their TG levels at pre-meal and post- The IMT and the endothelium-dependent vasodilatation (FMD) before and after lipofection were measured by ultrasound. Fifty elderly non-diabetic patients were selected as the control group. Results The levels of TG in observation group were significantly higher than those before meals (all P <0.05). TG levels in observation group were significantly higher than those in control group at each time point, the differences were statistically significant (P <0.05). The TG level of the control group at 2 h and 4 h after meal was significantly higher than that before meals (P <0.05), but returned to normal levels 6 h after the meal. The IMT in carotid arteries of the observation group was significantly higher than that of the control group (P <0.05), and the FMD of the observation group was significantly lower than that of the control group (P <0.05). The observation group and the control group There was no significant difference in carotid IMT at 4 h after operation (P> 0.05). FMD was significantly lower than that at fasting, the difference was statistically significant (P <0.05). Conclusion The elderly patients with postprandial diabetes mellitus elevated TG will have an impact on vascular endothelial function, can promote the occurrence of diabetes mellitus and cardiovascular diseases.
