血清Cys-C、hs-CRP与尿β2-MG、U-mAlb联合检测对糖尿病肾病的诊断价值

目的探讨血清高敏C反应蛋白(hs-CRP)、胱抑素(Cys-C)与尿β2-微球蛋白(β2-MG)、微量白蛋白(U-mAlb)联合检测在诊断糖尿病肾病中的临床意义。方法选取2015年8月至2016年11月于本院就诊的糖尿病患者72例,伴有糖尿病肾病的患者作为A组(n=36),单纯糖尿病患者作为B组(n=36);另选取同期体检健康者36例作为对照组。应用免疫比浊法测定受试者血清Cys-C及尿U-mAlb、β2-MG水平,酶联免疫吸附试验测定血清hs-CRP水平,比较各组hs-CRP、Cys-C、U-mAlb及β2-MG水平,计算hs-CRP、Cys-C、U-mAlb及β2-MG单项和联合检测的特异度、灵敏度及准确性。结果 B组hs-CRP、Cys-C、U-mAlb及β2-MG水平高于对照组,A组以上指标高于B组,差异均有统计学意义(均P<0.05);4项联合诊断的灵敏度高于hs-CRP、β2-MG单项检测,特异度高于hs-CRP单项检测,准确性高于hs-CRP、Cys-C、U-mAlb及β2-MG单项检测,差异均有统计学意义(均P<0.05)。结论 hs-CRP、Cys-C、U-mAlb及β2-MG联合诊断可进一步提高早期糖尿病肾病诊断的准确性,有助于缓解糖尿病肾病的发生发展,改善患者预后。 Objective To investigate the diagnostic value of serum hs-CRP, Cys-C and urinary β2-MG and U-mAlb in the diagnosis of diabetic nephropathy Clinical significance. Methods Totally 72 diabetic patients, diabetic nephropathy patients in group A (n = 36) and simple diabetic patients in group B (n = 36) were enrolled in our hospital from August 2015 to November 2016. Results The levels of hs-CRP, Cys-C, U-mAlb and β2-MG in group B were significantly higher than those in control group (P <0.05) The sensitivity was higher than hs-CRP, β2-MG single test, specificity higher than hs-CRP single test, the accuracy is higher than hs-CRP, Cys-C, U-mAlb and β2-MG single test, the difference was statistically Significance (both P <0.05). Conclusion The combined diagnosis of hs-CRP, Cys-C, U-mAlb and β2-MG can further improve the accuracy of diagnosis of early diabetic nephropathy, and help to alleviate the occurrence and development of diabetic nephropathy and improve the prognosis of patients.
