目的了解2型糖尿病(T2DM)患者糖化血红蛋白(HbA1c)控制情况,以及HbA1c达标与并发症、血脂、患者满意度依从性的相关关系。方法随机选取正在门诊进行降糖治疗的2型糖尿病患者490例,按HbA1c是否达标分为达标组(HbA1c≤6.5%,148例)和未达标组(HbA1c>6.5%,342例)。采用问卷调查的方式收集患者一般情况、患者满意度、依从性等相关资料。然后分别测定其糖化血红蛋白(HbA1c)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C),对该类患者血糖达标情况进行统计学描述和分析。结果符合入选条件患者共490例,HbA1c达标率为30.2%。未达标组中并发症发生率明显大于达标组(P<0.05)。达标组的血脂水平明显低于未达标组。达标组中患者的满意度及依从性明显大于未达标组(P<0.05)。结论目前2型糖尿病患者HbA1c达标率为30.2%,达标控制并不理想。HbA1c达标有利于减少并发症的发生,降低TC、TG、LDL-C水平。血糖达标组的患者较未达标组患者满意度高且依从性较好。
Objective To investigate the control of glycosylated hemoglobin (HbA1c) in type 2 diabetes mellitus (T2DM) and the relationship between HbA1c compliance and complication, blood lipid and patient satisfaction. Methods 490 patients with type 2 diabetes mellitus undergoing hypoglycemic treatment were randomly selected and divided into the standard group (HbA1c≤6.5%, 148 cases) and the non-compliance group (HbA1c> 6.5%, 342 cases) according to whether HbA1c was up to standard. Using questionnaires to collect the general condition of patients, patient satisfaction, compliance and other related information. Then the levels of HbA1c, TC, TG, HDL-C and LDL-C were determined respectively, and the blood sugar level of these patients was reached Statistical description and analysis. Results A total of 490 patients met the inclusion criteria, and the HbA1c compliance rate was 30.2%. The incidence of complications in the non-compliance group was significantly higher than that in the compliance group (P <0.05). The standard group of blood lipid levels were significantly lower than the non-compliance group. Satisfaction and compliance of the patients in the standard group were significantly higher than those in the non-compliance group (P <0.05). Conclusions The prevalence of HbA1c in patients with type 2 diabetes mellitus is 30.2% at present, and the compliance control is not satisfactory. HbA1c compliance is conducive to reduce the incidence of complications, reduce TC, TG, LDL-C levels. Patients in the standard glucose group were more satisfied and compliant than those who did not.
