目的探讨MR动态增强扫描联合DWI表观扩散系数在乳腺肿瘤中的诊断价值。方法选取2012年3月-2014年5月该院肿瘤科收治的54例经病理证实的乳腺肿瘤患者的临床资料进行回顾性分析,患者均经MR平扫、动态增强扫描和DWI检查,对比分析两者联合应用的准确性、敏感性及特异性。结果乳腺良恶性肿瘤平扫信号强度分布比较,差异无统计学意义(P>0.05);MR动态增强扫描诊断乳腺良恶性肿瘤的准确性为76.0%,敏感性为73.1%,特异性为75.0%;DWI表观扩散系数诊断准确性为77.8%,敏感性为76.9%,特异性为67.9%;联合诊断准确性为93.5%、敏感性为91.5%、特异性为92.1%,明显高于前两者(P<0.05)。结论 MR动态增强扫描联合DWI表观扩散系数在乳腺肿瘤中的诊断价值较高,其影像具有一定的特征性,能显著提高诊断准确性,提升对乳腺良恶性肿瘤的鉴别能力。
Objective To investigate the diagnostic value of dynamic contrast-enhanced MR combined with DWI apparent diffusion coefficient in breast neoplasms. Methods The clinical data of 54 patients with pathologically confirmed breast tumors admitted to the Department of Oncology from March 2012 to May 2014 in our hospital were retrospectively analyzed. All patients underwent MR scan, dynamic contrast-enhanced scan and DWI. Accuracy, sensitivity and specificity of the combination of the two. Results There was no significant difference in the signal intensity distribution between benign and malignant breast tumors (P> 0.05). The accuracy of MR dynamic contrast-enhanced scanning in diagnosing benign and malignant breast tumors was 76.0%, the sensitivity was 73.1% and the specificity was 75.0% The diagnostic accuracy of DWI was 77.8%, the sensitivity was 76.9% and the specificity was 67.9%. The accuracy of the combined diagnosis was 93.5%, the sensitivity was 91.5% and the specificity was 92.1%, which was significantly higher than the first two (P <0.05). Conclusions The diagnostic value of dynamic contrast-enhanced MR imaging combined with DWI apparent diffusion coefficient in breast neoplasms is high. The image has some characteristic features, which can significantly improve the diagnostic accuracy and improve the differential diagnosis of benign and malignant breast tumors.
