目的:探讨转子下截骨结合原位固定治疗股骨头骨骺滑脱的早中期临床疗效。方法:选取青少年股骨头骨骺滑脱患者5例,所有患者均行转子下截骨结合原位固定治疗。术后进行3个月、6个月、1年及1年以上随访,并行X线检查内固定情况,根据Harris评分对髋关节活动度进行综合评估。结果:所有患者均获得随访,随访时间1~1.6年,平均1.3年。所有患者髋膝关节疼痛症状逐渐消失,骨骺滑脱均未加重,髋关节Hariss由术前的(60.22±23.56)分提高至末次随访的(95.23±2.54)分。结论:转子下截骨结合原位固定能有效防止青少年股骨头骨骺滑脱进一步发展,缓解症状,改善生活质量,是治疗股骨头骨骺滑脱的有效手段,可避免远期股骨头坏死的发生。
Objective: To investigate the early and mid-term clinical efficacy of subrotate osteotomy combined with in situ fixation for the treatment of femoral epiphysis slippage. Methods: Five cases of adolescent femoral epiphyseal slippage were selected, all patients underwent subrotator osteotomy combined with in situ fixation. The patients were followed up for 3 months, 6 months, 1 year and more than 1 year after operation. X-ray examination and internal fixation were performed. The hip joint mobility was evaluated comprehensively according to Harris score. Results: All patients were followed up for 1 ~ 1.6 years with an average of 1.3 years. The symptoms of hip and knee pain disappeared in all patients. The epiphyseal slippage did not aggravate. The hip joint Hariss was increased from (60.22 ± 23.56) points to (95.23 ± 2.54) points at the last follow-up. CONCLUSIONS: Proximal osteotomy combined with in-situ fixation can effectively prevent further development of adolescent femoral epiphysis, relieve symptoms and improve quality of life. It is an effective treatment for the femoral epiphyseal slippage and can avoid the occurrence of long-term femoral head necrosis.
