目的探讨背心式高频振动排痰和机械振动排痰对老年肺部感染患者的疗效对比。方法选取30例老年肺部感染患者,随机分为高频组和机械组各15例。在常规治疗基础上,高频组采用背心式高频振动排痰仪,机械组采用机械振动排痰仪。观察两组在治疗前(D1)、治疗后第7 d(D7)、第14 d(D14)的呼吸频率(R)、末梢静脉氧饱和度(SpO_2)、日常生活能力(ADL)评分、住院时间、痰液情况等指标。结果两组日排痰量无明显差异,同一组中D7比D1,D14比D7的日排痰量比较,差异均有统计学意义。D7、D14的SpO_2、ADL评分比较,高频组高于机械组,差异有统计学意义(P<0.05)。组内比较,D7和D14分别与D1比较,R、ADL、SpO_2均有明显变化,差异有统计学意义(P<0.05)。D14与D7比较,除了机械组的ADL无明显差异外,其余各指标比较,差异均有统计学意义(P<0.05)。结论两种振动排痰方法均有效,痰量变化无差异,背心式高频振动排痰仪能较长时间改善咯痰难易度和痰液性状。该法能锻炼胸廓收缩力,提高血氧、缓解气促。
Objective To investigate the curative effect of vest-type high-frequency vibration sputum aspiration and mechanical vibration sputum aspiration on senile pulmonary infection. Methods Thirty elderly patients with pulmonary infection were selected and randomly divided into high frequency group and mechanical group in 15 cases. On the basis of routine treatment, high-frequency group using vest-type high-frequency vibration sputum instrument, the mechanical group using mechanical vibration sputum instrument. The respiratory rate (R), peripheral venous oxygen saturation (SpO_2), daily living ability (ADL) score of the two groups were observed before treatment (D1), 7th day after treatment (D7), 14th day Time, sputum and other indicators. Results There was no significant difference in sputum volume between the two groups. There was significant difference in the sputum output of D7 between D1 and D14 over D7 in the same group, the differences were statistically significant. D7, D14 SpO_2, ADL score, high-frequency group was higher than the mechanical group, the difference was statistically significant (P <0.05). There were significant differences in R, ADL and SpO_2 between D7 and D14 and D1 (P <0.05). D14 and D7, except for the mechanical group of ADL no significant difference, the other indicators, the differences were statistically significant (P <0.05). CONCLUSION Both the two methods of vibrating and expelling sputum are effective and there is no difference in the amount of sputum. The vest type high-frequency vibrating sputum can improve the susceptibility to sputum and the sputum for a long time. The law can exercise thoracic contractility, improve blood oxygen, ease the shortness of breath.
