目的:探讨检测生殖道分泌物和血清中基质金属蛋白酶(MMP-9)的含量对宫颈癌的诊断价值。方法:选取2010年1月~2013年12月该院妇产科收治的女性患者152名,其中宫颈癌92例为宫颈癌组,宫颈上皮内瘤变(CIN)60例为CIN组。另选择健康女性52例作为对照组。检测受试者生殖道分泌物和血清MMP-9的含量。结果:增生期、分泌期、排卵期、黄体期生殖道分泌物和血清中MMP-9含量比较,差异均无统计学意义(P>0.05)。宫颈癌组患者生殖道分泌物和血清MMP-9含量明显高于CIN组和对照组(P<0.01),CIN组生殖道分泌物和血清MMP-9含量明显高于对照组(P<0.01),宫颈癌组血清MMP-9水平和CIN组比较,差异均无统计学意义(P>0.05)。3组生殖道分泌物MMP-9水平均明显高于本组血清MMP-9水平(P<0.01)。宫颈癌不同分期Ⅰa期、Ⅰb期、Ⅱa期生殖道分泌物和血清中MMP-9浓度均有升高趋势,不同分期之间比较,差异均有统计学意义(P<0.01)。CINⅢ组生殖道分泌物、血清MMP-9水平高于CINⅠ组和CINⅡ组(P<0.01),后两组比较,差异均无统计学意义(P>0.05)。结论:女性不同生理周期的MMP-9含量无明显差异。宫颈癌、CIN患者生殖道分泌物MMP-9存在明显高表达,且CINⅢ患者生殖道分泌物和血清MMP-9水平明显高于CINⅠ~Ⅱ患者,宫颈癌不同分期患者生殖道分泌物和血清MMP-9水平存在明显差异,因此可通过检测生殖道分泌物MMP-9水平对宫颈癌和CIN进行早期筛查和诊断。
Objective: To investigate the detection of genital tract secretions and serum matrix metalloproteinase (MMP-9) levels of the diagnostic value of cervical cancer. Methods: A total of 152 female patients admitted to our department from January 2010 to December 2013 were enrolled in the study. Among them, 92 cases of cervical cancer were cervical cancer group and 60 cases of CIN were CIN group. Another 52 healthy women as a control group. Subjects were tested for genital secretions and serum levels of MMP-9. Results: There was no significant difference in the contents of MMP-9 in proliferative phase, secretory phase, ovulation period, luteal phase genital tract secretions and serum (P> 0.05). The contents of genital tract secretions and serum MMP-9 in patients with cervical cancer were significantly higher than those in CIN and control groups (P <0.01). The levels of genital secretions and serum MMP-9 in CIN group were significantly higher than those in control group (P <0.01) There was no significant difference in serum MMP-9 level between cervical cancer group and CIN group (P> 0.05). The levels of MMP-9 in genital tract secretions in the three groups were significantly higher than those in the group (P <0.01). The levels of MMP-9 in genital secretions and serum of Ⅰa stage, Ⅰb stage and Ⅱa stage of cervical cancer tended to increase, and there were significant differences between different stages (P <0.01). The level of MMP-9 in CINⅢgroup was higher than that in CINⅠgroup and CINⅡgroup (P <0.01). There was no significant difference between the two groups (P> 0.05). Conclusion: There was no significant difference in the MMP-9 levels in different female physiological cycles. Cervical cancer, CIN patients with genital secretions MMP-9 was significantly higher expression, and CIN Ⅲ patients with genital secretions and serum MMP-9 levels were significantly higher than CIN Ⅰ ~ Ⅱ patients with cervical cancer at different stages of reproductive tract secretions and serum MMP -9 levels there is a significant difference, so you can detect the level of MMP-9 of reproductive tract secretions of cervical cancer and CIN for early screening and diagnosis.
