In the past few years, some authors have studied epidermal alterations of lichen planus (LP). Their results support Kyrle’s (1925) concept of LP, in which the basal cell injury is the primary lesion of LP. However, the exact cause of so-called liquefaction necrosis or hyaline degeneration of basal cells and the exact link that causes changes in tissue biology are still not clear. The authors performed ultrastructural observations on biopsies from 15 LP patients with active papules and found many changes in the epidermis. As expected with light microscopy, the most obvious ultrastructural changes are in the area where the dermis binds to the epidermis: 1.
