目的通过锥形束CT三维成像研究单侧后牙正锁患者颞下颌关节的骨性特征。方法对35例安氏Ⅰ类单侧后牙正锁患者(实验组单侧后牙正锁患者19例,对照组安氏Ⅰ类无锁患者16例)使用口腔专用锥形束CT(CBCT)影像扫描。利用Ondemand3D软件中Dental Volume Reformat功能行三维重建,选择描述颞下颌关节骨性结构的9个点进行描记,对11个测量值进行分析统计。结果实验组锁侧与非锁侧比较颞下颌关节骨性形态差异有统计学意义(P<0.01);对照组左右两侧差异无统计学意义(P>0.05)。对照组与实验组锁侧和非锁侧分别比较,颞下颌关节骨性形态差异均有统计学意义(P<0.01)。结论 CBCT成像能有效分析颞下颌关节的骨性形态特征,单侧后牙正锁患者两侧髁突及关节窝具有形态学差异。
Objective To study the skeletal features of temporomandibular joint (JOJ) in patients with unilateral posterior locking positive locking by cone beam CT three-dimensional imaging. Methods 35 cases of Class Ⅰ unilateral posterior teeth positive lock patients (experimental group, one side of the posterior teeth positive lock patients 19 cases, control group, Class Ⅰ Lock patients 16 cases) using oral special cone beam CT (CBCT) image scan. Using Dedeng Volume Reformat function in Ondemand3D software to perform 3D reconstruction, nine points describing the skeletal structure of temporomandibular joint were selected for tracing, and 11 measurements were analyzed and statistically analyzed. Results There was significant difference in the bony morphological changes of temporomandibular joint between experimental group and control group (P <0.01). There was no significant difference between the left and right sides of the control group (P> 0.05). There were significant differences in the bony morphological changes of temporomandibular joint between control group and experimental group (P <0.01). Conclusion CBCT imaging can effectively analyze the bone morphological features of temporomandibular joint. There are morphological differences in the condyle and the joint between the two sides of the posterior teeth of the unilateral posterior locking.
