在一个海上石油平台上,操作人员正在用软管将一只移动式的化学品贮槽和一个贮罐连接起来,以输送甲醇。当吊车提起这只移动式贮槽,利用重力作用向贮槽进料时,甲醇通过软管上的洞喷溅出来,顶层甲板上的甲醇随后被点燃。喷溅到侧面的甲醇也被位于转移点正下方的二层甲板上的压缩机的高温排放气所点燃。一名工作人员试图用脚将贮槽上的阀门关闭,却打破了该槽上的视镜,火势愈加复杂。随着其它两个化学品贮槽的安全阀动作后,火势进一步扩大。在使用了22只13kg的手提化学灭火器,2只56kg的推车灭火器,以及消防水系统后,两个甲板上的火势被控制住。有一人遭受了2级烧伤。在大火被扑灭后,人们发现用于从贮槽向贮罐输送甲醇的软管裂开了,并且软管在使用前,曾用管道胶带修补过!
At an offshore oil platform, operators are using hoses to connect a mobile chemical tank to a tank to deliver methanol. When the crane lifts the mobile tank, gravity feeds the tank, methanol splashes through the holes in the hose, and the methanol on the top deck is subsequently ignited. Methanol splashed to the side is also ignited by the high temperature exhaust of the compressor on the decks located just below the transfer point. A staff member trying to foot the valve on the tank closed, but broke the mirror on the groove, the fire more complex. The fire was further amplified as the safety valves of the other two chemical tanks were activated. After using 22 13kg portable chemical fire extinguishers, two 56kg cart fire extinguishers, and a fire water system, the fires on both decks were contained. One suffered a grade 2 burn. After the fire was extinguished, it was found that the hose used to transport the methanol from the sump to the tank broke open and the hose was repaired with duct tape before use.
