目的 :观察适当增加套扎环的改良套扎法与硬化法联合治疗食管静脉曲张出血的疗效。方法 :将套扎与硬化法适当改良 ,即增加套扎环 ;首次治疗每例套扎 8～ 12环 ,复扎时 5～ 10点 ,每例次硬化 4～ 6点 ,在套扎点之间或套扎点上注射 5 %鱼肝油酸钠 ,该联合治疗法至曲张静脉消失或变细。结果 :15例病人经 31例次治疗 ,共结扎 2 6 9环。硬化 15 8点 ;曲张静脉消失 12例 (占 80 % ) ,变细 3例 (2 0 % ) ,总有效率 10 0 %。并发症轻微 ,胸痛 12例次 (38.7% ) ,发热 9例次 (2 9% ) ,未出现食管狭窄及穿孔等严重并发症。随访 1年无 1例出血。结论 :改良套扎及硬化法对食管静脉曲张出血是一种安全有效、副作用少的治疗方法。
Objective: To observe the curative effect of modified ligation method and sclerosis method to treat esophageal variceal hemorrhage with appropriate increase of ligation ring. Methods: The ligation and hardening method appropriate improvement, that is, increase the ligation ring; the first treatment of each case ligation 8 to 12 rings, 5 to 10 points in the complex, each case times hardened 4 to 6 points in the ligation point 5% injection of sodium morrhuate on the occasion of the laryngeal junction or between the combination therapy to varicose veins disappear or thinning. Results: Fifteen patients were treated with 31 cases and a total of 269 rings were ligated. Complications were mild, chest pain in 12 cases (38.7%), fever in 9 cases (29%), no serious complications such as esophageal stenosis and perforation. Follow-up 1 year without bleeding in 1 case. Conclusion: Improved ligation and sclerotherapy of esophageal variceal bleeding is a safe and effective, less side effects of the treatment.
