目的:研制开发腹主动脉瘤腔内隔绝术国产化器具。方法:以镍钛合金丝编织成Z形支架,外面包裹国产真丝超薄人造血管,装入导鞘制成腔内隔绝系统;以牛颈静脉间置法制作保留腰动脉的犬腹主动脉瘤模型;X线监视下将腔内隔绝系统经犬股动脉导入动脉瘤腔内,释放后将移植物固定在动脉瘤两端正常血管壁上,使动脉瘤与血流隔绝。结果:10只模型犬接受手术,成功9 只。术后即时检查,移植物与宿主动脉结合紧密,没有移位、内漏等并发症发生。1只因动脉破裂死亡。结论:国产腔内隔绝系统的质量、弹性、顺应性、实用性均满意,基本可以替代进口产品而应用于腹主动脉瘤腔内隔绝术。
Objective: To develop and develop localized instruments for endovascular exclusion of abdominal aortic aneurysms. Methods: Z-shaped stents were braided with nickel-titanium alloy wire. The super thin artificial blood vessels made of domestic silk were wrapped on the outer surface. The catheter was inserted into the endoluminal exclusion system. The dog abdominal aortic aneurysm with reserved lumbar artery was prepared by interposition of bovine jugular vein. Model; under the X-ray monitoring, the intraluminal exclusion system was introduced into the aneurysm cavity via the femoral femoral artery. After release, the graft was fixed on the normal vessel wall at both ends of the aneurysm, and the aneurysm was isolated from the blood flow. RESULTS: Ten model dogs received surgery and nine were successful. Immediately after the examination, the graft was tightly bound to the host artery and no complications such as displacement or endoleak occurred. One died of an arterial rupture. Conclusion: The quality, elasticity, compliance and practicality of the domestically produced endoluminal insula- tion system are satisfactory. It can be used as an alternative to imported products for endovascular exclusion of abdominal aortic aneurysms.
