有人认为,并行心律节奏点周围的保护性传入阻滞,其机理与位相阻滞有关,在早期是由3相阻滞所保护,在晚期则由4相阻滞所保护。我们最近遇到一例女性,56岁,冠心病病人,描记的一份间歇性室性并行心律的心电图(附图见封三),考虑其间歇性的机理是其并行灶周围仅存在3相保护性传入阻滞所致。现报告如下。附图中窦性心律下传之 QRS 波(R)形态正常,室性早搏之 ORS 波(R′)宽大畸形,形态相同,联律间距不等,短 R′—R′间距相等,提示为室性并行心律,但长 R′—′间距与短 R′—R′间距不成倍数,说明其长 R′—R′间并行灶的节律被打乱,又重建周期,使其成为间歇性。
It is believed that the protective afferent block around the rhythm point of the parallel heart rhythm is related to the phase retardation and is protected by the 3-phase block in the early stage and by the 4-phase block in the late stage. In the figure, QRS wave (R) in normal sinus rhythm is normal, and ORS wave (R ’) in normal premature ventricular contractions is large and deform, with the same shape and distance between RLC and R’-R’ Room parallel heart rhythm, but the long R’-spacing and short R’-R ’spacing is not a multiple, indicating that its long R’-R’ parallel focus between the rhythm was disrupted, but also to rebuild the cycle, making it intermittent.
