目的探讨小眼畸形转录因子(microphthalmia transcription factor,MITF)、c-Met和组织蛋白酶-K(cathepsin-K)在恶性黑色素瘤、痣和非典型性痣中的表达及意义。方法 32例恶性黑色素瘤(恶黑组)、13例普通痣患者(普通痣组)和6例非典型性痣患者(非典型痣组)的组织标本,采用免疫组织化学法检测3组组织中MITF、c-Met和cathepsin-K的表达情况。结果 MITF、c-Met表达于黑色素细胞的细胞核和细胞质,cathepsin-K表达于细胞质和细胞膜;MITF、c-Met和cathepsin-K阳性表达率恶黑组分别为93.8%、90.6%和93.8%,普通痣组分别为92.3%、92.3%和100.0%,非典型痣组分别为100.0%、100.0%和100.0%,3组间比较差异均无统计学意义(P>0.05);恶黑组MITF()、c-Met()及cathepsin-K()阳性率高于普通痣组和非典型痣组(P<0.05);恶黑组转移者MITF、c-Met及cathepsin-K阳性率明显高于非转移者(P<0.05),结节性恶性黑色素瘤者MITF、c-Met及cathepsin-K阳性率高于表浅扩散性恶性黑色素瘤者(P<0.05);恶黑组转移者MITF、c-Met及cathepsin-K阳性率均为100%,高于HMB-45阳性率(75.0%)(P<0.05)。结论 MITF、c-Met及cathepsin-K对恶性黑色素瘤的诊断及评估其进展和生物学行为有重要价值。
Objective To investigate the expression and significance of microphthalmia transcription factor (MITF), c-Met and cathepsin-K in malignant melanoma, nevus and atypical nevus. Methods Tissues of 32 patients with malignant melanoma (malignant melanoma group), 13 patients with common nevus (common nevi group) and 6 patients with atypical nevus (atypical nevus group) were detected by immunohistochemistry MITF, c-Met and cathepsin-K expression. Results MITF, c-Met were expressed in the nucleus and cytoplasm of melanocytes, and cathepsin-K was expressed in the cytoplasm and cell membrane. The positive rates of MITF, c-Met and cathepsin-K were 93.8%, 90.6% and 93.8% There was no significant difference between the three groups (P> 0.05). The levels of MITF (MITF) in the group of common nevi were 92.3%, 92.3% and 100.0%, and those in the group of atypical nevi were 100.0%, 100.0% and 100.0% (P <0.05). The positive rates of c-Met () and cathepsin-K () were higher in common nevus and atypical nevus (P <0.05). The positive rates of MITF, c-Met and cathepsin- (P <0.05). The positive rates of MITF, c-Met and cathepsin-K in nodular malignant melanoma were significantly higher than those in superficial diffuse malignant melanoma (P <0.05) The positive rates of MITF, c-Met and cathepsin-K were 100%, higher than that of HMB-45 (75.0%) (P <0.05). Conclusion MITF, c-Met and cathepsin-K have important value in diagnosis and evaluation of malignant melanoma and their biological behavior.
