目的:进一步探讨超声引导下微波介入治疗肝癌可行性和疗效。方法:应用 U M C- Ⅰ超声引导微波治疗仪,对43 例中晚期肝癌进行了研究,采用分次多点多部位由深到浅低功率长时间分段凝固,输出功率30~60 W ,作用时间200~1 800 s。结果:所有病例治疗后肿块缩小,其中3 例完全消失,肿块中央回声增强,周边呈低回声,境界清晰,肿瘤内血流消失或减少,12 例 C T 检查,肿块缩小,无强化,23 例超声引导下活检大片坏死。43 例均健在。随访最长 12 月,治疗后全身情况好转,症状减轻或消失,病人饮食明显增加(33 例), A F P和肝功能改善,生活质量提高。无严重并发症发生。结论:超声引导微波对中晚期肝癌的治疗有较好的疗效,安全实用,适于各期肝癌的治疗。
Objective: To further explore the feasibility and efficacy of ultrasound-guided microwave interventional therapy for liver cancer. METHODS: Ultrasonography-guided microwave therapy instrument U M C- I was used to study 43 cases of advanced hepatocellular carcinoma. The segmented multi-points and multi-parts were used for deep-to-light and low-power segmented coagulation for a long time. The output power was 30-60 W. Working time 200 ~ 1 800 s. Results: In all cases, the mass was reduced after treatment. Three cases completely disappeared. The central echo of the mass was enhanced. The periphery was hypoechoic and the realm was clear. The blood flow in the tumor disappeared or decreased. Twelve cases had CT examination, and the mass was reduced. No enhancement was observed in 23 cases. Ultrasound-guided biopsy of large necrosis. 43 cases are alive. After up to 12 months of follow-up, the general condition improved after treatment, the symptoms were reduced or disappeared, the patient’s diet was significantly increased (33 cases), AFP and liver function improved, and the quality of life improved. No serious complications occurred. Conclusion: Ultrasound-guided microwave has good curative effect on the treatment of advanced liver cancer. It is safe and practical, and it is suitable for the treatment of liver cancer in various stages.
