目的 探讨三维(3D) CT扫描矢状重组评估腰椎滑脱(LS)节段椎间盘退变的价值.方法 分析162例LS患者的3DCT扫描资料,在矢状重组像作下位椎体后上角与上位椎体后下及后上角连线L1、L2;于椎间盘上终板平面上,以椎间盘后缘与L1、L2不相交者为正常,与L1相交为膨隆,与L2相交为突出.结果 162例中,3DCT扫描矢状重组诊断椎间盘突出16例,14例经手术病理证实,总符合率为87.50%.其中,12例椎间盘后缘与L2相交,2例与L1相交.结论 采用3DCT扫描矢状重组显示LS节段椎间盘后缘与L1、L2的关系评估椎间盘退变程度时,与病理诊断的符合率高,有助于提高LS节段椎间盘退变的影像学诊断.“,”Objective To explore the role of 3DCT scan sagittal reconstruction in evaluating degeneration of intervertebral disc in lumber spondythelysis(LS).Methods The data of 3DCT scan from 162 cases with LS were analyzed by drawning a line connecting the upper and trailing edge of the lower vertebral body and the lower and trailing edge of the upper vertebral body with L1 and L2.At the upper plane of intervertebral disc end plate,the normal intervertebral disc was diagnosed when the line did not cross with the L1 and L2 vertebrae,and intervertebral disc bugle and hemiation were defined as the line crossed with the L1 and L2 vertebrae,respectively.Results Of 162 cases,16 cases were diagnosed as intervertebral disc herniation on 3DCT scan sagittal reconstruction,of whom 14 cases were confirmed pathologically after surgery with an accordance rate of 87.50%.The line crossed with L2 vertebrae in 12 cases and crossed with L1 vertebrae in 2 cases.Conclusion To evaluate the degree of degeneration of intervertebral disc of LS patients with analysing the relationship between its trailing edge and L1 and L2 vertebrae has a high accordance rate with pathological diagnosis,which can provide an effective image diagnosis in LS patients with intervertebral disc lessions.
