Step 1: Defining Questions Ensure that issues of concern are fully understood and clearly expressed. Describe the causes and consequences: What triggered the unwillingness to see the consequences. For example: frequent equipment failure caused delays and quality problems of delivery. Clear the impact of the problem on the results.
