【目的】观察地塞米松静脉滴注预防国产紫杉特尔导致的化学性静脉炎的临床效果。【方法】2005年7月至2006年2月期间,将使用紫杉特尔化疗的入院病人按照随机数字表随机分为A、B两组:A组(应用地塞米松预防紫杉特尔所致的静脉炎)16例;B组(常规使用紫杉特尔化疗)23例。选择2004年7月至2005年7月使用单药顺铂同期化疗的病人作为历史对照组C组(常规单药顺铂化疗)23例。B、C两组采用浅静脉滴注常规化疗。A组在上述的基础上,应用紫杉特尔化疗前后给予生理盐水100 ml+地塞米松5 mg静脉滴注10 min~15 min,同时停止口服紫杉特尔化疗当天常规预处理使用的地塞米松。【结果】对导致静脉炎发生的Logistic回归分析显示:组别是影响静脉炎发生的独立因素。A、B、C三组的静脉炎发生率分别为31.2%,82.6%,17%(P<0.05)。各组间两两比较,A组的静脉炎发生率明显低于B组(P<0.05),而和C组比较,则无统计学差异。发生的静脉炎分型比较中得出:A组的红肿型、硬结型、坏死型发生率分别为31.3%,6.3%,0%,均低于B组(47.8%,52.2%,43.3%),两组间三型静脉炎的比较,存在统计学差异(P<0.05)。【结论】地塞米松静滴能有效减少紫杉特尔所致的化学性静脉炎的发生,减轻静脉炎发生的程度。
【Objective】 To observe the clinical effect of intravenous dexamethasone in preventing chemical phlebitis induced by domestic taxotere. 【Methods】 During the period from July 2005 to February 2006, patients admitted with chemotherapy using taxotere were randomly divided into A and B groups according to a random number table: group A (dexamethasone for the prevention of taxotere Caused by phlebitis) in 16 cases; B group (routine use of taxotere chemotherapy) in 23 cases. The incidence of phlebitis in group A was significantly lower than that in group B (P <0.05), but there was no significant difference between group A and group C (P> 0.05). The incidences of phlebitis in group A were 31.3%, 6.3% and 0% respectively, which were lower than those in group B (47.8%, 52.2% and 43.3%, respectively) There was a significant difference between the three groups in phlebitis (P <0.05). 【Conclusion】 Dexamethasone intravenous infusion can effectively reduce the incidence of chemical phlebitis caused by taxifolin and reduce the incidence of phlebitis.
