目的:评价海藻酸钠-聚赖氨酸-海藻酸钠(APA)微囊化牛肾上腺嗜铬细胞(BCC)移植于患者蛛网膜下腔后的镇痛效果。方法:分4个剂量组:微胶囊0.5×107,1.0×107,1.25×107,1.5×107/(次·例)。采用腰椎穿刺术,将生理盐水稀释后的微胶囊注入20例中、重度癌痛患者L3~4或L4~5蛛网膜下腔内。结果:移植后第1~5天开始起效,移植前疼痛强度(数字分级法)为7.8±1.5,移植后降至2.5±2.3(P<0.05)。镇痛总有效率在0.5×107,1.0×107,1.25×107,1.5×107/(次·例)剂量组分别为:66%,100%,80%,66%。镇痛持续中位时间24d,最长持续时间>120d。移植后患者生活质量评分较移植前升高10分(P>0.05)。结论:APA-BCCs微胶囊蛛网膜下腔移植治疗中、重度癌痛具有起效较快、长效镇痛的特点。
OBJECTIVE: To evaluate the analgesic effect of alginate-polylysine-alginate (APA) microencapsulated bovine adrenal chromaffin cells (BCC) transplanted into the subarachnoid space of patients. Methods: The four dose groups: microcapsules 0.5 × 107, 1.0 × 107, 1.25 × 107, 1.5 × 107 / (times · cases). Using lumbar puncture, the physiological saline diluted microcapsules into 20 cases of moderate and severe cancer patients L3 ~ 4 or L4 ~ 5 subarachnoid space. Results: The first day to the fifth day after transplantation started to take effect. The intensity of pain before transplantation (digital grading method) was 7.8 ± 1.5 and decreased to 2.5 ± 2.3 after transplantation (P <0.05). The total effective rate of analgesia was 66%, 100%, 80% and 66% in the dose groups of 0.5 × 107, 1.0 × 107, 1.25 × 107 and 1.5 × 107 / (times · cases) respectively. Analgesia continued median time 24d, the longest duration of> 120d. The quality of life of patients after transplantation increased by 10 points compared with that before transplantation (P> 0.05). Conclusion: APA-BCCs microcapsule subarachnoid transplantation in the treatment of moderate and severe cancer pain has a rapid onset, long-term analgesic characteristics.
