目的:观察芪明颗粒联合常规药物治疗糖尿病视网膜病变(DR)的临床疗效。方法:选取90例DR患者为研究对象,按随机数字表法分为对照组和研究组各45例。对照组给予常规药物治疗,研究组在常规药物治疗基础上加用芪明颗粒治疗。观察2组临床疗效,比较2组视力、眼底出血、黄斑水肿的改善情况及血液流变学的变化。结果:研究组总有效率为88.9%,高于对照组的68.9%,差异有统计学意义(P<0.05)。治疗后研究组的视力、眼底出血和黄斑水肿改善率分别为88.9%、91.1%和95.6%;对照组的视力、眼底出血和黄斑水肿改善率分别为68.9%、71.1%和75.6%。2组3项改善率比较,差异均有统计学意义(P<0.05)。治疗前,2组血浆黏度、红细胞压积、红细胞变形指数以及纤维蛋白原等血液流变学指标水平比较,差异均无统计学意义(P>0.05)。治疗后,对照组的血浆黏度较治疗前降低(P<0.05);研究组的血浆黏度、红细胞压积、纤维蛋白原水平均较治疗前降低(P<0.05),红细胞变形指数较治疗前升高(P<0.05);研究组血浆黏度、红细胞压积、纤维蛋白原水平均低于对照组(P<0.05),红细胞变形指数高于对照组(P<0.05)。结论:芪明颗粒联合常规药物治疗DR临床疗效显著,能有效提高患者的视力,改善患者眼底出血、黄斑水肿等状况以及血液流变学指标。
Objective: To observe the clinical effect of shenming granule combined with routine medicine on diabetic retinopathy (DR). Methods: Ninety DR patients were selected as the research object and randomly divided into control group and study group (n = 45) according to random number table method. Control group given conventional drug treatment, the study group on the basis of conventional drug therapy plus stilbene particles treatment. The clinical curative effect of the two groups were observed. The improvement of visual acuity, fundus hemorrhage, macular edema and hemorheology were compared between the two groups. Results: The total effective rate of the study group was 88.9%, which was higher than that of the control group (68.9%), the difference was statistically significant (P <0.05). The improvement rates of eyesight, fundus hemorrhage and macular edema in the study group after treatment were 88.9%, 91.1% and 95.6%, respectively. The improvement rates of eyesight, fundus hemorrhage and macular edema in the control group were 68.9%, 71.1% and 75.6% respectively. There were statistically significant differences between the two groups in 3 improvement rates (P <0.05). Before treatment, there was no significant difference in plasma viscosity, hematocrit, erythrocyte deformability index and hemorrheological indexes such as fibrinogen between the two groups (P> 0.05). The plasma viscosity, hematocrit and fibrinogen of the study group were lower than those before treatment (P <0.05), and the index of erythrocyte deformity was higher than that before treatment (P <0.05) (P <0.05). The plasma viscosity, hematocrit and fibrinogen of the study group were lower than those of the control group (P <0.05). The index of erythrocyte deformability was higher than that of the control group (P <0.05). Conclusion: The combination of Qiming granule and conventional medicine has significant clinical curative effect on DR, which can effectively improve the vision of patients and improve the fundus hemorrhage, macular edema and other indicators of hemorheology.
