目的分析单核细胞趋化蛋白-1(MCP-1)、血清铁与老年糖尿病性骨质疏松的相关性。方法选取糖尿病合并骨质疏松患者43例(DO组),糖尿病无骨质疏松的患者42例(DM组)。检测2组患者腰椎(L1~L4)及左侧股骨颈部的骨密度,并采集空腹血检测2组患者血清铁及外周血中MCP-1蛋白质表达水平。结果 DO组腰椎T值和股骨颈T值均低于DM组,血清铁低于DM组,血清MCP-1高于DM组,差异均有统计学意义(P<0.05)。血清铁与股骨颈T值(r=0.613,P<0.05)、腰椎T值(r=0.592,P<0.05)呈正相关;MCP-1表达水平与股骨颈T值(r=-0.85,P<0.05)、腰椎T值(r=-0.89,P<0.05)呈负相关。结论糖尿病性骨质疏松外周血MCP-1表达水平增高,血清铁含量减少,MCP-1和血清铁可能参与了糖尿病性骨质疏松的发生。
Objective To analyze the relationship between MCP-1, serum iron and senile diabetic osteoporosis. Methods Forty-three patients with diabetes mellitus and osteoporosis (DO group) and 42 patients with osteoporosis without diabetes (DM group) were enrolled in this study. The bone mineral density (BMD) of lumbar vertebrae (L1 ~ L4) and left femur neck were measured in two groups. The expression of MCP-1 protein in serum and peripheral blood were detected in two groups of fasting blood samples. Results The lumbar spine T value and femoral neck T value in DO group were lower than those in DM group. The serum iron level was lower in DM group than in DM group. The serum MCP-1 level was higher in DM group than in DM group (P <0.05). Serum iron and femoral neck T value (r = 0.613, P <0.05), lumbar spine T value (r = 0.592, P <0.05) was positively correlated; MCP-1 expression and femoral neck T value (r = -0.85, P < 0.05) and lumbar spine T value (r = -0.89, P <0.05). Conclusion The expression of MCP-1 in peripheral blood of diabetic patients with osteoporosis is increased, and serum iron content is decreased. MCP-1 and serum iron may be involved in the pathogenesis of diabetic osteoporosis.
