众所周知,医疗卫生事业直接关系到我国亿万人民的身体健康,是重大的民生事业,其在近几年的发展过程中也取得了长足的进步。但是,虽然我国公立医院可持续发展的经济来源有一部分是医院自身的收入,但主要依靠的还是政府的财政补贴。然而,随着新医改政策及相关配套措施的逐步实施,“以药补医”的补偿模式正在发生变化,医院补偿机制暴露出了诸多的问题与漏洞,而其中最为突出的问题即在于医疗费用的快速膨胀,民众医药费用的自付比例明显过高,医疗负担日益沉重,甚至出现了“因病致贫因病返贫”的现象。基于此,关于公立医院补偿机制的改革已经势在必行。本文首先简要阐述了我国公立医院补偿机制的现状,其次对新医改政策下公立医院对补偿机制进行改革的意义及难点进行了系统的分析与探讨,并提出了相应的改革措施。
As we all know, the medical and health undertaking is directly related to the health of hundreds of millions of our people. It is a major cause of livelihood and has made great strides in the development of recent years. However, although part of the economic source of sustainable development of public hospitals in our country is the hospital’s own income, it mainly depends on the government’s financial subsidies. However, with the gradual implementation of the new health care reform policy and related supporting measures, the compensation model of “supplement medicine by doctors” is changing. Many problems and loopholes have been exposed in the hospital compensation mechanism. The most prominent problem is that The rapid expansion of medical costs, public medical expenses significantly higher proportion of out-of-pocket payment, the medical burden is becoming heavier and heavier, and even the phenomenon of “causing poverty due to illness causes poverty”. Based on this, the reform of public hospital compensation mechanism has been imperative. This paper first briefly states the status quo of the compensation mechanism in public hospitals in our country, secondly, it analyzes and discusses the significance and difficulties of the reform of the compensation mechanism in public hospitals under the new policy of medical reform, and puts forward corresponding reform measures.
