目的通过分析2005-2007年湖南省各市(州)网络直报疑似肺结核病人报告现状,评价结核病防治机构与非结核病防治机构的合作和协调情况。方法对近3年结核病信息管理系统湖南省各市(州)非结核病防治机构网络直报疑似肺结核病人的肺结核病例报告卡,剔除重报病例和已到结核病防治机构就诊的病例,获取未到结核病防治机构就诊病人的相关信息,分析其转诊到位率、追踪率及总体到位率变化及其原因。结果2005-2007年湖南省非结防机构网络直报肺结核病人数分别是26 521例,38 840例和57 305例,排除重报和辖区外患者数,则应转诊人数分别是20 623例,33 288例和47 649例。2005-2007年转诊到位率分别是45.23%,39.02%,28.83%;追踪到位率分别是49.76%,47.02%,53.82%;总体到位率分别是68.19%,65.90%,64.97%。结论湖南省各市(州)非结核病防治机构网络直报疑似肺结核病人转诊率、追踪率没有达到国家的要求。需加强综合医院医生培训与宣传,提高结防机构和非结防机构的合作,以提高结核病的发现水平。
Objective To assess the status of cooperation between TB prevention and treatment agencies and non-TB prevention and treatment agencies by analyzing the status quo of direct reporting of suspected TB patients from all cities and counties in Hunan Province during 2005-2007. Methods To report the tuberculosis case report card of suspected tuberculosis patients to the network of non-tuberculosis prevention and control institutions of cities and counties in Hunan Province in the past three years for TB information management system, and to report the case report of tuberculosis cases and those who have reached the TB prevention and treatment institutions, Agencies visit the patient’s relevant information, analysis of the referral rate, follow-up rate and the overall rate of arrival and its causes. Results The number of direct reports of tuberculosis in non-TB prevention and cure institutions in Hunan Province from 2005 to 2007 was 26,521, 38,840 and 57,305, respectively. The numbers of patients who were excluded from reporting and area of jurisdiction were 20,623 , 33 288 cases and 47 649 cases. The referral rates from 2005 to 2007 were 45.23%, 39.02% and 28.83%, respectively. The tracking rates were 49.76%, 47.02% and 53.82% respectively. The overall occupancy rates were 68.19%, 65.90% and 64.97% respectively. Conclusion The network of non-TB prevention and treatment institutions of all cities (prefectures) in Hunan Province directly reported the referral rate of suspected TB patients, and the follow-up rate did not meet the national requirements. It is necessary to strengthen the training and publicity of doctors in general hospitals so as to enhance the cooperation between prevention and control agencies and non-prevention agencies in order to raise the level of detection of tuberculosis.
