目的探讨Nd:YAG激光治疗人工晶状体眼继发恶性青光眼的疗效。方法回顾性分析24例(24只眼)人工晶状体眼继发恶性青光眼病例,分别采用药物联合Nd:YAG激光治疗行晶状体后囊膜及玻璃体前界膜切开术。结果Nd:YAG激光治疗后,所有病例前房都有不同程度的加深,眼压下降,视力部分恢复。每个病例平均进行1~3次激光反复治疗,其中3只眼因前房恢复不理想而采用前部玻璃体切除治疗。随访时间14个月(1~36个月),患者前房稳定,眼压控制。结论正确选择药物联合激光治疗行晶状体后囊膜及玻璃体前界膜切开术,对治疗人工晶状体眼继发恶性青光眼是非常有效的。具有损伤小、痛苦少、恢复快、操作简便、可多次进行等特点。此技术可作为人工晶状体眼继发恶性青光眼的治疗方法之一。
Objective To investigate the effect of Nd: YAG laser in the treatment of secondary glaucoma caused by intraocular lens. Methods A retrospective analysis of 24 patients (24 eyes) with intraocular lens (IOL) secondary to malignant glaucoma was performed. Drugs combined with Nd: YAG laser were used to treat the posterior capsule and anterior vitrectomy. Results Nd: YAG laser treatment, all cases of anterior chamber have different degrees of deepening, intraocular pressure decreased visual acuity partially restored. On average, 1 to 3 laser treatments were performed in each case, with anterior vitrectomy in 3 of the eyes due to suboptimal recovery of the anterior chamber. Follow-up time of 14 months (1 to 36 months), patients with anterior chamber stability, intraocular pressure control. Conclusion The correct choice of drugs combined with laser treatment of posterior capsular lens and anterior vitreous membrane incision, for the treatment of intraocular lens secondary malignant glaucoma is very effective. With less damage, less pain, fast recovery, easy to operate, can be repeated and so on. This technique can be used as an intraocular lens in the treatment of secondary malignant glaucoma.
