目的探讨不同血型与脑卒中的关系。方法采用病例-对照研究方法,以脑卒中住院患者1987例为病例组,其中脑梗塞1016例,脑出血755例,蛛网膜下腔出血216例;选择35岁以上非脑卒中住院患者21051例为对照组,计算各型脑卒中ABO血型的四种表现型的构成比,分析不同表现型与各型卒中的关系。结果(1)脑梗塞组,四种表现型构成比:O型36.4%、A型31%、B型25.8%、AB型6.8%;比值比(OR):O型0.952[95%可信区间CI 0.835~1.085]、A型1.066(95%CI 0.931~1.222)、B型1.012(95%CI 0.876~1.168)、AB型0.936(95%CI 0.729~1.201),P均>0.05。(2)脑出血组,构成比:O型42.5%、A型27%、B型24.2%AB型6.2%;OR:O型1.229(95%CI 1.061~1.424),P<0.05,A型0.879(95%CI 0.746~1.034)、B型0.932(95%CI0.787~1.103),AB型0.853(95%CI 0.632~1.150),P>0.05。(3)蛛网膜下腔出血组,构成比:O型39.4%、A型28.7%、B型26.9%、AB型5.1%;OR:O型1.078(95%CI 0.820~1.419)、A型0.955(95%CI 0.711~1.285)、B型1.069(95%CI 0.790~1.446),AB型0.689(95%CI 0.375~1.267),P均>0.05。结论ABO血型与脑卒中无相关性,脑出血组O型血的OR值有显著意义,提示脑出血与O型血可能有一定关联。
Objective To explore the relationship between different blood groups and stroke. Methods A case-control study was conducted in 1987 hospitalized stroke patients, including 1016 cases of cerebral infarction, 755 cases of cerebral hemorrhage and 216 cases of subarachnoid hemorrhage. 21051 cases of non-stroke hospitalized over 35 years old were selected as In the control group, the proportions of the four phenotypes of ABO blood group in each type of stroke were calculated, and the relationship between different phenotypes and each type of stroke was analyzed. (2) The intracerebral hemorrhage group, the constituent ratio: O type 42.5%, A type 27%, B type 24.2% AB type 6.2%; OR: O type 1.229 (95% CI 1.061-1.424), P < (95% CI 0.746-1.034), type B 0.932 (95% CI 0.787-1.103), type AB 0.853 (95% CI 0.632-1.150), P> 0.05. (3) Subarachnoid hemorrhage group: the ratio of O type 39.4%, A type 28.7%, B type 26.9%, AB type 5.1%; OR: O type 1.078 (95% CI 0.820-1.419), A type 0.955 (95% CI 0.711-1.285), B type 1.069 (95% CI 0.790-1.446), AB type 0.689 (95% CI 0.375-1.267), all P> 0.05. Conclusion There is no correlation between ABO blood group and stroke. The OR value of O blood group in cerebral hemorrhage group is significant, suggesting that there may be some correlation between cerebral hemorrhage and O blood group.
