目的探讨显微外科技术治疗颅底脑膜瘤的手术方法,评估手术效果。方法在气管插管全麻下采用相应恰当的手术入路,在显微镜直视下操作,妥善处理肿瘤与血管、神经、静脉窦及脑干的关系,分块切除肿瘤。手术全切肿瘤36例,近全切11例,大部分切除10例。术后辅以放疗。结果治愈41例。术后死亡3例。结论认真的术前评估,正确选择颅底入路,显露充分,仔细的术中技术,手术医师的经验是取得全切除和良好结果的重要因素。
Objective To investigate the surgical treatment of skull base meningiomas by microsurgery and evaluate the effect of surgery. Methods The appropriate surgical approach under general anesthesia with endotracheal intubation was operated under the microscope. The relationship between the tumor and the blood vessels, nerves, sinus and brain stem was properly treated. The tumor was divided and removed. Totally 36 cases of tumor were completely resected and 11 cases were completely resected. Most cases were resected in 10 cases. Postoperative radiotherapy. The results were cured in 41 cases. 3 cases died after operation. Conclusions Serious preoperative assessment, proper choice of cranial base approach, revealing adequate and careful intraoperative techniques, and surgeon experience are important factors in achieving total resection and good outcome.
