目的⑶以彩色多普勒血流显像分析了 75 例原发性肝癌 ⒉1 05 个病灶⒕和 5 4 例肝转移癌 ⒉96 个病灶⒕的血供类型⒙旨在提供诊断和鉴别诊断的依据⒚方法⑶用彩色多普勒血流频谱对肝固有动脉、门静脉和肿瘤的供养血管的血流动力学参数进行测定⒙所用指标为最大血流速度 ⒉ Vm ax⒕⒛阻力指数 ⒉ R I⒕⒛搏动指数 ⒉ P I⒕⒛最大与最小血流速度之比 ⒉ A / B⒕和血流量 ⒉ F V ⒕⒚结果⑶原发性肝癌和肝转移癌的肝固有动脉和门静脉的 Vm ax 和 F V 均高于正常对照组 ⒉ P< 00 01⒕⒙而肝固有动脉的 R I、 P I、 A/ B 值均低于对照组⒚原发性肝癌组中 82 个病灶可见搏动性血流信号 ⒉9 11% ⒕⒙其中 65 个为 ⒈篮网⒔状 ⒉722% ⒕⒚肝转移癌主要表现为瘤周供血⒚结论⑶彩色多普勒血流显像有助于提高原发性肝癌和肝转移癌的诊断和鉴别诊断精度⒙但对于多血管性肝转移癌的鉴别是不可靠的⒚
