目的探讨外周血淋巴细胞亚群对健康成人基因重组乙肝疫苗免疫效果的影响,阐明重组乙肝疫苗免疫失败的机理。方法根据基因重组乙肝疫苗接种后产生抗-HBs的水平分为免疫应答组和免疫无(弱)应答组,随机选取每组其中的60例作为研究对象,利用EPICS XL型流式细胞分析仪对研究对象外周血淋巴细胞亚群CD3+、CD4+、CD8+进行分析。结果健康成人接种基因重组乙肝疫苗后产生免疫应答组和无(弱)应答组的CD3+、CD4+、CD8+T细胞数在两组间的百分比分别为66.5±8.4、33.5±5.9、34.0±6.5和67.1±8.7、24.9±6.6、24.5±5.9。统计学分析结果表明,CD4+、CD8+T细胞数在两组间存在显著性差异(P<0.001),但CD3+T细胞在两组间无明显差别(P>0.05)。结论健康成人外周血CD4+、CD8+T细胞的数量及功能可能影响基因重组乙肝疫苗的免疫效果,而T淋巴细胞总数影响不大。
Objective To investigate the effect of peripheral blood lymphocyte subsets on the immunogenicity of recombinant hepatitis B vaccine in healthy adults and to elucidate the mechanism of immune failure of recombinant hepatitis B vaccine. Methods According to the level of anti-HBs produced by gene recombinant hepatitis B vaccination, the immune response group and immune non-weak (weak) response group were randomly divided into two groups. 60 of them were randomly selected as the research object. The EPICS XL flow cytometry The subjects were analyzed for peripheral blood lymphocyte subsets CD3 +, CD4 +, CD8 +. Results The percentages of CD3 +, CD4 + and CD8 + T cells in immunocompetent and non-attenuated group were 66.5 ± 8.4,33.5 ± 5.9,34.0 ± 6.5 and 67.1 ± 8.7, 24.9 ± 6.6, 24.5 ± 5.9. The results of statistical analysis showed that the number of CD4 + and CD8 + T cells was significantly different between the two groups (P <0.001). However, CD3 + T cells showed no significant difference between the two groups (P> 0.05). Conclusion The number and function of CD4 +, CD8 + T cells in peripheral blood of healthy adults may affect the immune effect of recombinant hepatitis B vaccine, while the total number of T lymphocytes has little effect.
