Introduction While transfusion-transmitted diseases are increasingly receiving attention in the field of transfusion medicine, there has been no apparent improvement in how to minimize transfusion-related errors. A literature analysis of blood transfusion accidents inconsistent with ABO blood type showed that there has been no apparent change in the transfusion rate of ABO blood type in the past 40 years (from 1/10,000 to 1/4 million infusion of blood units), although it has been adopted in recent years. A large number of detection methods, including a blood alert system, are in place to prevent the fatal potential errors described above, but human error has been observed to account for 50% of transfusion-related errors.
