报告48例重型脑外伤患者血儿茶酚胺(CA)测定结果及其中13例肺活检光、电镜观察所见,35例正常人血CA作对照组。结果表明:外伤组血CA明显高于对照组。死亡组病人伤后24小时内和第3天血CA均高于存活组(P<0.01)。肺脏病理改变主要为肺泡萎陷,上皮细胞及血管内皮细胞肿胀变性,肺泡间隔及肺泡腔水肿,细胞浸润等。死亡组病变明显重于存活组,两组间尤以超微结构病理改变差别显著。结合文献讨论了重型脑外伤急性期血CA变化与肺脏病理改变的关系及其临床意义。
Report of 48 cases of severe traumatic brain injury in patients with catecholamine (CA) determination results and 13 cases of lung biopsy light seen by electron microscopy, 35 cases of normal human blood CA control group. The results showed that: blood CA level in trauma group was significantly higher than that in control group. The levels of serum CA in patients in the death group within 24 hours and on the third day after injury were significantly higher than those in the surviving group (P <0.01). Pulmonary pathological changes mainly for alveolar collapse, epithelial cells and endothelial cell swelling and degeneration, alveolar septa and alveolar edema, cell infiltration. The death group was significantly more than the survival group, pathological changes between the two groups especially ultrastructural differences were significant. Combined with the literature discussed the acute phase of severe traumatic brain injury and its relationship with the pathological changes of lung pathological changes and its clinical significance.
